摘要
目的研究p53、Ki-67、E-cadherin和CD34在胆囊癌中的表达及临床意义。方法手术切除的共胆囊组织120例,分为3组:胆囊癌组、癌旁组和正常组,各40例,采用免疫组化法分别检测三组的p53、Ki-67、E-cadherin和CD34的表达情况,并进行相关性分析。结果正常组、癌旁组和胆囊癌组中p53的阳性表达率分别为0,12. 5%和72. 5%,Ki-67的阳性表达率分别为2. 5%、17. 5%和80. 0%,E-cadherin的阳性表达率分别为95. 0%,85. 0%和30. 0%,3组间比较,差异有统计学意义(P <0. 05); CD34(MVD均数)在胆囊癌组织(57. 43±5. 21)的表达均高于正常组(30. 68±4. 59)和癌旁组(32. 66±4. 28),差异具有统计学意义(P <0. 05)。免疫组织化学法结果显示,p53、Ki-67、E-cadherin和CD34阳性表达与胆囊癌的分化程度和淋巴结转移均具有明显的相关性;与病人的年龄、性别、肿瘤直径和肿瘤部位中均无相关性(P> 0. 05)。在TNM分期中,仅CD34(MVD均数)表现出明显差异(P <0. 05),p53、Ki-67、E-cadherin与TNM分期不具有相关性(P> 0. 05)。结论 p53、Ki-67、E-cadherin和CD34与胆囊癌细胞的侵袭转移密切相关,但仍需临床大数据研究的进一步验证。
Objective To study the expression and clinical significance of p53,Ki-67,E-cadherin and CD34 in gallbladder carcinoma. Methods We collected a total of 120 cases of cholecystectomy,which were divided into 3 groups: 40 cases of gallbladder carcinoma( gallbladder cancer group),40 cases of paracancerous tissue( paracancer group),and 40 cases of normal gallbladder tissue( normal group). Immunohistochemistry was used to detect the expression of p53,Ki-67,E-cadherin and CD34 in the tissues.Results The positive expression rates of p53 in the normal group,paracancer group,and gallbladder cancer group were 0,12. 5% and 72. 5%,respectively. The difference between the three groups was statistically significant( P < 0. 05). The positive expression rates of Ki-67 were 2. 5%,17. 5% and 80%,respectively( P < 0. 05). The positive expression rates of E-cadherin were 95. 0%,85. 0% and 30. 0%,respectively( P < 0. 05). The expression of CD34( MVD mean) in gallbladder carcinoma tissue( 57. 43 ±5. 21) was higher than that in normal group( 30. 68 ± 4. 59) and paracancer group( 32. 66 ± 4. 28),the difference was statistically significant( P < 0. 05). The results of immunohistochemistry showed that the positive expression of p53,Ki-67,E-cadherin and CD34 were significantly correlated with the degree of differentiation and lymph node metastasis of gallbladder carcinoma. There was no correlation with the patient’s age,gender,tumor diameter and tumor location,and the difference was not statistically significant( P > 0. 05). In the TNM stage,only CD34( MVD mean) showed significant difference( P < 0. 05),p53,Ki-67,E-cadherin and TNM staging were not correlated( P > 0. 05). Conclusion p53,Ki-67,E-cadherin and CD34 are closely related to the invasion and metastasis of gallbladder cancer cells,but further validation by clinical big data research is still needed.
