摘要 目的探讨人工血管在血液透析通路中的临床应用,相关并发症及处理方法。方法 2012年1月-2017年6月,在动静脉内瘘术中行人工血管移植,建立血液透析通路共76例,术后随访分析相关并发症及处理方法。结果进行人工血管移植的患者中,6个月通畅率为94.74%,12个月通畅率为89.47%,24个月通畅率为80.26%。并发症中以血栓形成后闭塞多见,占28%,心衰为12%,感染为8%,假性动脉瘤形成为6%,其他并发症为2%。结论在血管条件差、无法完成自体动静脉内瘘术时,以人工血管移植建立透析通路,有维护方便、容易穿刺、远期通畅率高、透析流量充足等优点,只要我们不断提升手术熟练程度,术后科学维护,及时合理的处理术后并发症,人工血管移植建立透析通路将成为提高慢性肾脏病患者生存质量的重要方法。 Objective To investigate the clinical application complications and corresponding processing method of artificial vascular graft in constructing hemodialysis access.Methods We created an arteriovenous fistula for hemodialysis access by using an artificial vascular graft in 76 patients between January 2012 and June 2017.Postoperatively,the patients were followed up for any complications.Results Patency rate at 6,12 and 24 months were 94.74%,89.47%and 80.26%,respectively.The most frequently reported complication was postthrombotic occlusion(28%),followed by heart failure(12%),infection(8%),pseudoaneurysm(6%),and others(2%).Conclusion In cases where an autologous arteriovenous fistula could not be created,the use of artificial vascular graft for constructing hemodialysis access may be an alternative option.With advantages such as easy maintainance and a high long-term patency rate,this method is expected to have a great potential in improving the quality of life of patients with chronic kidney disease.
