髌骨爪结合克氏针张力带治疗髌骨下极骨折 被引量:2

Effect of patellar claw combine with Kirschner tension -band wirein treating inferior fracture of patella

摘要 目的:分析髌骨爪结合克氏针张力带治疗髌骨下极骨折的临床疗效。方法:2013年1月-2017年6月我院采用髌骨爪结合克氏针张力带治疗髌骨下极骨折患者21例,作为观察组。24例采用单纯克氏针张力带治疗,作为对照组。记录2组患者术后骨折愈合时间、并发症发生情况,依据HSS评分标准评定末次随访时膝关节功能。结果:45例患者均获随访,随访时间9-17个月,平均(11±0.7)个月。观察组骨折愈合时间(9.48±0.42)周,对照组骨折愈合时间(10.55±0.35)周,观察组早于对照组,差异有统计学意义(P<0.05)。观察组术后出现创伤性关节炎1例;对照组术后出现张力带松动2例,关节僵硬1例,创伤性关节炎1例,2组并发症发生率差异有统计学差异(P<0.05)。末次随访时观察组膝关节功能HSS评分优良率为90.5%,对照组为83.3%,观察组高于对照组,差异有统计学差异(P<0.05)。结论:髌骨爪结合克氏针张力带治疗髌骨下极骨折更加符合生物力学原则,并发症少,膝关节功能恢复好。 Objective:To report the clinical efficacy of patellar claw combined with Kirschner tension -band wire in the treatment of inferior polar fracture of patella.Methods:From January 2013 to June 2017,21 patients with patella fractures were treated with patellar claw combined with Kirschner tension -band wire as group A.24 patients were treated with Kirschner tension -band wire as group B. The postoperative fracture healing time and complications were recorded in the two groups.The knee function was evaluated at the last follow -up according to the HSS score.Results:All 45 patients were followed up for 9 to 17 months,with an average of(11±0.7)months. The fracture healing time was (9.48±0.42)weeks in group A,and the fracture healing time was (10.55±0.35 )weeks in group B.Group A was shorter than group B,and the difference was statistically significant (P <0.05 ).1 patient developed traumatic arthritis in group A, there were 2 cases of tension band loosening,1 case occurred joint stiffness,and 1 cases of traumatic arthritis in group B,there was significant difference in the incidence of complications between the two groups (P <0.05 ).At the last follow -up,the excellent rate of HSS scores in group A was 90.5%,and that in group B was 75%.Group A was higher than group B,and the difference was statistically significant(P <0.05).Conclusion:Patellar claw combined with Kirsehner tension -band wire in the treatment of inferior polar fracture of patella was recommended because it had the advantages of biomechanical principles,less complications,and good knee function recovery.
