摘要
目的 总结211例大型听神经瘤手术治疗效果,探讨大型听神经瘤手术治疗策略。方法 211例大型听神经瘤采用枕下乙状窦后经内听道入路,在面神经监护下行显微手术切除肿瘤,术后随访时间3个月至5年。结果 肿瘤全切196例(93％);近全切除7例(3.3％);大部切除8例(4％)。面神经解剖保留192例(91％)。结论 大型听神经瘤手术治疗是首选的治疗方法,术中面神经监护对保留面神经完整性非常重要。
Objectives To summarize the experience of surgical treatment and discuss the strategy of surgical treatment for the huge acoustic neuromas. Methods Two hundred and eleven patients with huge acoustic neuroma were treated by microneurosurgery, during which facial nerve monitoring was performed, via suboccipital retrosigmoid and transmeatal approach. All the patients were postoperatively followed up for 3 months to 5 years. Results Of 211 cases of the huge acoustic neuromas, 196 were totally resected (93%), 7 subtotally (3.3%)and 8 in greater part (4%). Facial nerves kept anatomic intact during the operation in 192 patients (91%) with the huge acoustic neuromas. Conclusions Surgery treatment is the best choice for the huge acoustic neuromas. The intraoperation facial nerve monitoring is important to preserving the facial nerve function in the patients with huge acoustic neuromas.
