嗜麦芽寡养单胞菌感染及其耐药性分析 被引量:7

摘要 目的 :了解嗜麦芽寡养单胞菌感染的临床及耐药性特点。方法 :1997年 7月~ 2 0 0 0年 6月从临床送检标本分离 114株嗜麦芽寡养单胞菌 ,对其进行抗菌药物敏感试验和诱导型 β-内酰胺酶测定。结果 :114例嗜麦芽寡养单胞菌感染病例中 ,肺部感染为 5 8.77% ,泌尿系感染为 2 5 .4 4 %。该菌对氨苄西林、羧苄西林、头孢唑林、头孢呋辛、头孢曲松、头孢他啶、庆大霉素、阿米卡星的耐药率均在 6 0 %以上 ,对亚胺培南的耐药率几乎为 10 0 % ,但对替卡西林 /克拉维酸、环丙沙星、SMZ/ TMP的耐药率在 30 %以下。其诱导型β-内酰胺酶产生率为 6 3.16 %。结论 :治疗嗜麦芽寡养单胞菌感染 ,可选 SMZ/ TMP、环丙沙星或替卡西林 /克拉维酸。 It is reported that Lincomycin A is composed of an amino acid subunit, PHA and a sugar subunit, MTL. The proposed metabolic precursors of PHA and MTL are L tyrosine and Glucose respectively. The biosynthetic pathway of lincomycin was used for rational selection of lincomycin producing strain Streptomyces lincolnensis var lincolnensis . Then the mycelia were treated with UV (253.7nm, 30w, 120s) and NTG (2mg/ml), we selected the glucose tolerant mutant on the agar plants containing 18% glucose; the L tyrosine tolerant mutant on the agar plants containing 30% L tyrosine; and the lincomycin resistance mutant strains on the agar plants containing lincomycin 12000μg/ml. Results showed: that the rate of the titer positivie mutants are 60%~70% in glucose toleranence and lincomycin resistance methods and 20% higher than the rates in random selected methods. Finally a strain 00 282 was obtained from above methods, its potency increased by 50% over the parent strain 98 1, while the glucose tolerance increased by 20%, and lincomycin resistance increased to 14000μg/ml. The storage time of the strain 00 282 was prolonged to 3 weeks with its titre maintained over 95%, and the titre of F 3 remained stable at 95% initial level, meanwhile NTG was more efficient mutagent than UV120s in this experiment.
