摘要
目的 :探讨腰椎上关节面方位及对腰段脊柱稳定性的影响。方法 :测量 45套成人腰椎上关节面边缘的相互距离 ,并计算出上关节面的方位角。结果 :腰椎上关节面的方位角自上而下由大逐渐变小 ,男性右侧L1为 69.9°± 10 .5° ,L5为 61.5°± 10 .0° ,女性右侧L1为 66.4°± 8.5° ,L5为 60 .2°± 17.8° ,同一节段上关节面的方位角右侧大于左侧 ,相邻节段间无显著性差异 ;男性L1～L4节段左右两侧方位角比较 ,有显著性差异 (t=2 .15～ 4.43 ,P <0 .0 5 ) ,女性仅L4节段左右两侧方位角比较 ,有显著性差异 (t =2 .2 5 ,P <0 .0 5 ) ;男女两性方位角比较 ,仅L4节段左侧方位角有显著性差异 (t =2 .0 1,P <0 .0 5 )。结论 :腰椎上关节面的方位由近矢状位逐渐过渡到近冠状位 。
Objective:To observe the orientational angle of articular surface of lumbar vertebra and study its significance in the stability of spine.Methods:The length of each articular surface as well as the disatances between the anterior and posterior points of surface were measured on 45 sets of adult lumbar vertebra specimens and the orientational angles were calculated.Results:The orientational angles of upper articular surface of lumbar vertebra gradually become smaller downwards(for example,male(right):L 169.9°±10.5°,L 5 61.5°±10.0°;female(right):L 1 66.4°±8.5°,L 5 60.2°±17.8°).In each vertebra,the right orientational angle was larger than the left.There was no significant difference between the neighbor level ,but it has significant difference between the two sides of male from L 1 to L 4 (t=2.15～4.43,P< 0.05) and female's L 4 (t=2.25,P <0.05). It also has sexual difference on the left of L 4 (t=2.01,P <0.05). Conclusions:The orientation of upper articular surface of lumbar vertebra gradually transmits from coronary to sagittal position.The orientational angles of the upper two articular surface of lumbar vertebrae are asymmetry.
