摘要
目的探讨血压参数对伴高血压的抑郁症患者记忆功能的影响。方法对193例伴高血压的抑郁症患者采用行为记忆测验第二版(RBMT-Ⅱ)、汉密尔顿抑郁量表(HAMD)评定记忆功能和抑郁程度。结果伴高血压的抑郁症患者记忆障碍发生率为88.60%。高血压分级不同的抑郁症患者记忆障碍的比例不同(χ2=9.785,P<0.01),且不同分级的患者记忆标准总分、回忆姓名、路线延迟回忆、脸部再认、回忆被藏物品和定向评分间差异均有统计学意义(P<0.05);相关分析结果显示,收缩压、脉压与RBMTⅡ标准总分均呈负相关(P<0.01),与舒张压无相关性(P>0.05);多元线性逐步回归分析结果显示,RBMTⅡ标准总分低与患者年龄大、脉压高、抑郁程度重、受教育年限少有关。结论血压、抑郁程度均影响伴高血压的抑郁症患者的记忆功能。
Objective To explore the impact of blood pressure parameters on memory function in depressed patients with hypertension. Methods 193 depressed patients with hypertension were assessed with Rivermead Behavioural Memory Test- Second Edition (RBMT-Ⅱ ) and Hamilton Depression Scale (HAMD) to evaluate the memory function and severity of depression. Results Incidence rate of memory dysfunction was 88.60%. There was significant difference in incidence rate of memory dysfunction among groups with different grade of hypertension (X2 = 9. 785, P 〈 0. 01 ). There were significant differences in total score of RBMT-Ⅱ, as well as in factor scores of name recalling, delayed route recalling, face recognition, stored articles recalling and orientation among groups with different grade of hypertension ( P 〈 0.05 ). Correlation analysis indicated that total score of RBMT-Ⅱ total score was negatively correlated with systolic pressure and pulse pressure (P 〈 0. 01 ). There was no significant correlation between total score of RBMT- I1 and diastolic pressure (P 〉 0.05). Multiple linear stepwise regression analysis indicated that low total score of RBMT-Ⅱ was associated with advanced age, high pulse pressure, high severity of depression, low level of education. Conclusion Both blood pressure and severity of depression affect memory function in depressed patients with hypertension.
