摘要
目的:观察比较喹硫平合并米氮平治疗精神分裂症阴性症状的疗效和不良反应。方法:92例以阴性症状为主的精神分裂症患者,随机分为2组,观察组45例给予喹硫平+米氮平,对照组47例给予利培酮+米氮平。在治疗前及治疗后2,4,8周以阳性与阴性症状量表(PANSS)评定疗效,治疗过程中采用不良反应量表(TESS)观察安全性。结果:与治疗前比较,两组治疗2,4,8周PANSS总分及因子分均有显著下降(P均<0.05);治疗2周后,观察组和对照组在情感迟钝、情感退缩两个因子分值开始明显降低(P<0.01),观察组的情感交流障碍、被动/淡漠两个因子分值也明显降低(P<0.05),而对照组这两个因子分值是治疗4周后才明显降低(P<0.05);治疗8周后,两组患者的抽象思维困难、交谈缺乏自发性这些因子分值均明显降低(P<0.05)。两组的临床疗效、有效率及不良反应发生率差异均无统计学意义(P>0.05)。结论:喹硫平联合米氮平与利培酮联合米氮平治疗精神分裂症阴性症状疗效和安全性相当。
Objective：To observe the efficacy and adverse reactions of quetiapine combined with mirtazapine in the treatment of schizophrenia with negative symptoms.Methods：Totally 92 cases of schizophrenia predominantly with negative symptoms were randomly divided into two groups,the study group was treated by quetiapine combined with mirtazapine,and the control group was given risperidone combined with mirtazapine.The efficacy and adverse reactions were evaluated according to the Positive and Negative Symptom Scale （PANSS） and Treatment Emergent Symptom Scale （TESS） before the treatment and at the end of the 2nd,4th,and 8th week after the treatment.Results：Compared with those of pre-treatment,PANSS total scores and factor scores of the two groups were obviously decreased at the end of the 2nd,4th,and 8th week （P ＜ 0.05）.After the 2-week treatment,the factor scores of affective dullness and emotional withdrawal of the two groups were notably decreased （P ＜ 0.01）,furthermore,the factor scores of emotional disorder,passiveness and indifference of the study group were also decreased significantly（P ＜ 0.05）,while those of the control group were decreased after the 4-week treatment（P ＜ 0.05）.After the 8-week treatment,the factor scores of abstract thinking difficulties and lack of spontaneity of conversation of the two groups were decreased obviously （P ＜ 0.05）.There were no significant differences in the efficacy and adverse reactions between the two groups （P ＞ 0.05）.Conclision：The efficacy and safety of quetiapine combined with mirtazapine are similar with those of risperidone combined with mirtazapine in the treatment of schizophrenia with negative symptoms.
