外科手术患者多药耐药菌感染的原因与对策 被引量:3

Analyze of causes and countermeasures of multi-drug resistant bacterial infections for surgical patients

摘要 目的探讨医院手术患者多药耐药菌感染的原因,并提出控制手术后多药耐药菌感染的对策,指导临床控制患者多药耐药菌感染的发生。方法选取2010-2012年在医院外科进行手术1 656例患者,将其分为感染组140例和未发生感染组1 516例;分析多药耐药菌感染的比例,对比多药耐药菌感染与非多药耐药菌感染的外科手术后医院感染患者之间的因素,研究多药耐药菌感染的因素。结果 3年医院外科1 656例手术患者发生感染140例,医院感染率为8.45%;3年多药耐药菌感染所占比例在不断上升,经检验3年来多药耐药菌感染例数占感染例数的比例差异有统计学意义(P<0.05);医院手术后多药耐药菌感染的原因主要有:患者的年龄大、住院时间长、合并其他重大疾病、进行侵入性操作、留置尿管时间长、留置腹腔引流管和院外不规范治疗等。结论医院手术患者多药耐药菌感染的发生率逐年升高,医护人员应缩短患者住院时间,并加强对易感人群的护理,加强宣教并控制手术患者多药耐药菌感染的发生。 OBJECTIVE To analyze the causes of multi-drug resistant bacterial infections for surgical patients, and to explore countermeasures for preventing multi-drug resistant bacterial infections. METHODS Totally 1656 patients who underwent surgeries in the year 2010 to 2012 were chosen and were divided into the infection group (n= 140) and the non-infection group (n= 1516). The proportion of infections with multi-drug resistant bacteria was analyzed. The causes of multi-drug resistant bacterial infections were analyzed via comparison of factors for multi-drug resistant bacterial infections and nonmultidrug resistant bacterial infections after surgeries. RESULTS There were 140 cases of infections in the 1656 surgical patients during the three years, indicating the rate of hospital infections was 8. 45%. The proportion of multi-drug resistant bacteria kept rising during the three years. According to the test, the difference in the proportion of case numbers of multi-drug resistant bacterial infection to the number of hospital infections was significant (P〈0.05). And the old age of patients, duration of hospitalization, complication with other major diseases, invasive operation, long catheter application time, indwelling drainage tube and nonstandard treatment out of the hospital were the causes of multi-drug resistant bacterial infections. CONCLUSION As the occurrence rate of multi-drug resistant bacterial infection was increasing year by year, the medical staff should strengthen the nursing of susceptible population, shorten the hospitalization time, enhance education and control the occurrence of multidrug resistant bacterial infections.
