摘要
[目的]观察解郁合剂联合西药辨证分型治疗抑郁症疗效。[方法]使用随机平行对照方法,将160例住院患者按随机数字表分为两组。对照组80例氟西汀,20mg/d。治疗组80例解郁合剂(柴胡、知母、百合各15g,夏枯草、郁金、炒白芍各12g,牡丹皮、炒栀子各10g,甘草6g;肝郁气滞加香附6g,青皮4g;肝郁脾虚加党参15g;肝血郁滞加川芎、丹参及红花各10g;肝郁痰结加胆南星、瓜蒌各l0g;心脾两虚加白术、黄芪各15g;肝肾阴虚加山茱萸、生地黄各10g;脾肾阳虚加肉苁蓉15g),1剂/d,水煎100mL,一次顿服;氟西汀治疗同对照组。连续治疗20d为1疗程。观测临床症状、抑郁情绪评价、躯体症状评价、不良反应。连续治疗2疗程,判定疗效。[结果]治疗组痊愈34例,显效30例,有效12例,无效4例,总有效率95.00%;对照组痊愈15例,显效20例,有效27例,无效17例,总有效率78.75%。治疗组疗效优于对照组(P<0.05)。HAMD、SDS、SCL-90评分两组均有明显降低(P<0.05,P<0.01),治疗组降低优于对照组(P<0.05,P<0.01)。[结论]解郁合剂联合氟西汀辨证分型治疗抑郁症,疗效满意,无副作用,值得推广。
[ Objective ] To investigate Jieyu decoction combined with western medicine in differentiation treatment of depression syndrome. [ Methods ] 160 patients with depression syndrome were chosen in recent years in our hospital and randomly divided into two groups including control group （ 80 patients ） with fluoxetine for 20mg/d and treatment group （ 80 patients ） with Jieyu decoction for 1 dose/d on the basis of control group with continuous treatment for 20d in 2 groups ; and the clinical symptoms, depression mood, somatic symptoms and adverse reactions of both groups were compared. [ Results ] In treatment group, 34 cases were cured, 30 cases were markedly effective, 12 cases were effictive, 4 cases were no effect, and the total effective rate was 95.00%. In control group, 15 cases were cured, 20 cases were markedly effective, 27 cases were effietive, 17 cases were no effect, and the total effective rate was 78.75%.The clinical improving efficacy of treatment group was significant higher than control group （P〈0.05） . The scores of HAMD, SDS and SCL-90 of both groups after treatment was significant higher than before treatment （P〈0.05） . The improving degree of treatment group in scores of HAMD, SDS and SCL-90 was significant better than control group （ P〈0.05 ） . [ Conclusion ] Jieyu decoction combined with western medicine in differentiation treatment of depression syndrome can efficiently relieve depression emotions with satisfactory effects and no adverse effects.
