摘要
目的：探讨原位置换经外周静脉穿刺中心静脉置管（PICC）在老年患者中应用的可行性。方法选取行 PICC 需重新置管老年患者26例，均给予原位置换 PICC，观察患者的穿刺成功率，局部渗血、血肿形成等出血性并发症，以及静脉炎等导管相关并发症情况。结果所有患者均成功置换导管，X 线全胸片提示 PICC 导管末端位于 T6～T7水平下缘，位置正确。其中1次成功24例，2次成功2例，平均留置导管时间6～12个月，所有患者均未出现重度出血并发症、静脉血栓形成、空气栓塞等相关并发症。结论原位置换 PICC 操作简单、创伤小，可安全有效地应用于老年患者。
Objective To explore the feasibility of the in situ displacement of peripherally inserted central catheter （PICC）in elderly patients.Methods A total of 26 patients with PICC who needed new catheters were given in situ displacement of PICC.Successful rate of puncture, hemorrhagic complications （local errhysis and swelling formation）and catheter-associated complications （phlebitis）were observed.Results All patients were successfully displaced catheters,and X-ray showed the end of PICC catheters were placed rightly on the inferior margin of T6~T7 levels, in which 24 patients were successes by the first surgery and 2 by the second one.The mean catheter stays was 6~12 months,and no patients had relevant complications like severe hemorrhagic complications,venous thrombosis and air embolism,etc.Conclusion In situ displacement of PICC is simple in operation and slight in trauma,which can be effectively and safely applied in elderly patients.
