摘要
目的:探讨Th22细胞在乙肝相关性原发性肝癌(PHC)患者外周血的表达水平及临床意义。方法:收集16例乙肝肝硬化、14例乙肝相关性PHC合并肝硬化患者和16例健康对照者外周血,荧光实时定量聚合酶链反应(RT-PCR)检测血清乙型肝炎病毒(HBV)DNA载量,流式细胞术检测三组外周血Th22、Th1、Th17和Treg细胞所占比例,并逐层分析比较。结果:Th22和Th17细胞在PHC组明显高于肝硬化组和对照组(P<0.05),肝硬化组高于对照组(P>0.05)。Th1细胞在PHC组和肝硬化组明显高于对照组(P<0.05),其中PHC组低于肝硬化组(P>0.05)。Treg细胞PHC组、肝硬化组和对照组依次下降(P<0.05)。肿瘤直径与Th17和Th22细胞呈显著正相关(P<0.05),与Th1和Th17细胞无显著相关性(P>0.05)。HBV DNA与Th1、Th17、Th22和Treg细胞均无显著相关性(P>0.05)。结论:Th22细胞可能与Th1、Th17和Treg细胞一样,均参与PHC的发生和进展,且与乙肝病毒复制情况无关,其作用机制可能与Th17细胞类似或通过Th17细胞发挥作用。
Objective:To investigate the expression of Th22 cells in peripheral blood of primary hepatic carcinoma patients infected with hepatitis B virus (HBV) and its clinical significance. Methods: Peripheral blood of 16 patients with liver cirrhosis, 14 patients with cirrhotic primary hepatic carcinoma and 16 healthy controls, all of patients were infected with HBV. Serum HBV DNA levels were measured by fluorescence real-time quantitative polymerase chain reaction (RT-PCR), and the percentages of Th22, Th1, Th17 and Treg cells were assessed by flow cytometry(FCM). Results: The percentages of Th17 and Th22 cells in PHC patients were significantly higher than that in liver cirrhosis patients and healthy controls ( P 〈 0. 05 ), and The percentages of Th1 cells in PHC patients and liver cirrhosis were significantly higher than that in healthy controls (P〈0.05), and the level in PHC patients were lower than that in liver cirrhosis patients ( P 〉 0. 05 ). The percentages of Treg cells were decreased successively in PHC patients, liver cirrhosis patients and healthy controls(P〈0. 05). In PHC patients, The tumor diameter was positively correlated significantly with the percentages of Thl7 and Th22 cells (P〈0. 05 ), but not with that of Thl and Treg cells (P〉0. 05 ). Serum HBV DNA was not correlated signifinicantly with the percentages of Th22, Thl ,Th17 and Treg cells (P〉0.05). Conclusion: Like Thl, Th17 and Treg cells, Th22 ceils may also function in PHC patients inflected with HBV , and the functions do not have any correlations with HBV DNA. Th22 cells may contribute to the pathogenesis and disease progression of PHC in the same way with Thl7 ceils or through Thl7 cells.
