摘要
目的 观察2型糖尿病合并脑梗死患者凝血功能的改变,探讨其水平变化的意义.方法 检测糖尿病合并脑梗死组、糖尿病无合并脑梗死组及正常对照组的D-二聚体、凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)和纤维蛋白原(FIB)指标并比较.结果 三组间TT比较差异无统计学意义(P>0.05);糖尿病有/无合并脑梗死组与正常对照组PT比较差异有统计学意义(P<0.05);糖尿病无合并脑梗死组与正常对照组APTT、FIB和D-二聚体比较差异无统计学意义(P>0.05),而糖尿病有/无合并脑梗死组间、糖尿病合并脑梗死组与正常对照组APTT、FIB和D-二聚体比较差异有统计学意义(P<0.01).心功能Ⅰ~Ⅱ级APTT、FIB、D-二聚体水平明显低于心功能Ⅲ~Ⅳ级,差异有统计学意义(P<0.01).结论 凝血功能检测可用于糖尿病有无脑梗死合并症发生的界定,特别对心功能级别不同的糖尿病合并脑梗死患者,其检测指标可反映心功能水平.
Objective To observe the changes of coagulation function of patients with diabetes combined with cerebral infarction and discuss its significance. Methods D-dimer, prothrombin time (PT), activated partial thromboplastin time (APTT) ,thrombin time(TT) and fibrinogen(FIB) of the group with diabetes combined with cerebral infarction, the group with diabetes combined with no cerebral infarction group and the normal control group were observed and compared.Results Differences of TT in the three groups were not statistically significant( P 〉0.05 ).PT of the groups with diabetes was significantly different from that of the normal control group( P 〈0.05).The APTF,FIB and D-dimer of the group with diabetes combined without cerebral infarction were not significantly different from those of the normal control group ( P 〉0.05 ) .The APTT, FIB and D-dimer of the group with diabetes combined with cerebral infarction were significantly different from those of the group with diabetes combined without cerebral infarction and those of the normal control group ( P 〈0.01).The levels of APTT, FIB and D-dimer in I- II level of cardiac function were significantly lower than those in the III- IV level. Conclusion Detection of coagulation function can be used to define complications of cerebral infarction in diabetic patients. Especially for patients with diabetes combined with cerebral infarction of different cardiac function levels, the detection index can reflect patients' cardiac function levels.
