摘要
目的:探讨鼻咽癌肿瘤组织中信号转导和转录激活因子3(STAT3)的表达与鼻咽癌放疗效应的关系,评估STAT3在预测鼻咽癌放疗效应中的价值,为制订个性化的治疗方案提供理论依据。方法:对40例放射敏感和28例放射抵抗的鼻咽癌患者放疗前肿瘤组织中STAT3的蛋白表达情况应用免疫组织化学法进行检测,对STAT3的表达强度及其与放疗效应的关系进行分析,对STAT3在预测鼻咽癌放疗效应的价值进行评估。结果:STAT3在鼻咽癌中总的阳性表达率为100%。在放射治疗敏感的鼻咽癌组织中,9例(22.5%)患者STAT3表达强阳性,31例表达弱阳性;在放射抵抗组,21例(75%)患者STAT3表达强阳性,7例表达弱阳性,两组间表达强度STAT3的差异有统计学意义(χ2=18.414,P<0.01)。根据STAT3阳性表达预测鼻咽癌放疗抵抗,敏感性为75.00%,特异性为77.50%,准确率为76.47%,阳性预测值为70.00%,阴性预测值为81.58%,假阳性率为30.00%,假阴性率为18.42%。结论:鼻咽癌肿瘤组织中STAT3的表达强度与肿瘤放射抵抗有关,STAT3对预测鼻咽癌放疗效应有一定的价值,并据此制订出个性化的治疗方案。
Objective：To investigate the relation between the expression of STAT 3 protein and the effect of radiother-apy, estimate the value of STAT 3 to predict the effect of radiotherapy in patients with nasopharyngeal carcinoma, provide theoretical basement to establish characteristic project of treatment. Method：The expression of STAT 3 protein was exam-ined in 40 radiosensitive and 28 radioresistant poorly differentiated NPC by immunohistochemical staining （SP method）. None of patients had received radiotherapy and chemotherapy. The association of the expression of STAT 3 and the effect of radiotherapy was analyzed. The significance of STAT 3 was evaluated to predict the effect of radiotherapy in patients with nasopharyngeal carcinoma. Result：The positive rates of STAT 3 protein expressions were 100 % in NPC tissues. The rate of STAT 3 strong postive expressions was 75.00% and 22.25% in radioresistant and radiosensitive NPC tissues, re- spectively. It was statistical difference. Based the expression of STAT 3 to predict the effect of radiotherapy, Sensitivity, specificity, accuracy, positive predictive value,negative predictive value, false positive and false negative were 75.00%, 77.50%, 76.47%, 70. 00% ,81.58% ,30.00% and 18.42%, respectively. Conclusion：The strong postive expressions of STAT 3 were correlated with the radioresistant of NPC. STAT 3 expression may serve as a marker for predicting response to radiotherapy in NPC. According to the expression of STAT 3, the sound treatment scheme can be selected.
