N端脑钠肽原检测在充血性心力衰竭诊断中的应用 被引量:1

摘要 目的 分析并评价充血性心力衰竭患者血清N端脑钠肽原检测的临床应用价值.方法 选择2010年9月至2012年6月武警后勤学院附属医院收治的130例充血性心力衰竭患者作为心力衰竭组,另选择同期门诊进行健康体检者42例作为正常对照组,分别在治疗前及1个月后对充血性心力衰竭患者及正常对照组进行血清中N端脑钠肽原水平的测定.结果 心力衰竭组治疗前[(1795.1±519.2) ng/L]及治疗后[(723.9±224.2)ng/L]血清N端脑钠肽原水平均显著高于对照组[(65.7±27.4) ng/L或(65.9±27.7) ng/L],差异有统计学意义,且心功能分级NYHAⅡ~Ⅳ级组间血清N端脑钠肽原水平比较均有统计学意义(P<0.01).NYHAⅡ~Ⅳ级充血性心力衰竭患者经治疗后血清N端脑钠肽原水平[(723.9±224.2) ng/L]均比治疗前[(1795.1±519.2) ng/L]显著下降.结论 血清N端脑钠肽原水平可以反映出充血性心力衰竭患者的病情严重程度以及病情变化情况,可作为病情诊断、评价疗效及预后的主要参考指标. Objective To analyze and evaluate the clinical effect of congestive N terminal B type natriuretic peptide detection in the diagnosis of heart failure patients serum. Methods A total of 130 cases of patients with congestive heart failure admitted in the Affiliated Hospital of Logistics College of Chinese People's Armed Police Force from Sep. 2010 to Jun. 2012 were selected as congestive heart failure group, another 42 cases of healthy people underwent physical examination during the same period were selected as control group,for the serum N terminal B type natriuretic peptide levels of the two groups before treatment and one month after treatment were detected. Results In congestive heart failure group, the serum N terminal B type natriuretic peptide levels before and after treatment were (1795.1 ±519.2 ) ng/L and (723.9 ± 224.2 ) ng/L,which were significantly higher than (65.7 ±27.4) ng/L and (65.9 ±27.7) ng/L of the control group,differences were statistically significant, and serum N terminal B type natriuretic peptide levels in cardiac functional grading NYHA Ⅱ -Ⅳgroups showed statistically significant differences. Moreover, serum N terminal B type natriuretic peptide levels of NYHA Ⅱ -Ⅳ congestive heart failure patients significantly decreased after treatment (723.9 ±224.2 ) ng/L than before treatment ( 1795.1 ± 519.2 ) ng/L, there was statistically significant difference. Conclusion Serum N terminal B type natriuretic peptide level of patients with congestive heart failure may reflect the severity of the disease and the change of disease, which can be used as the main reference index for diagnosis,therapeutic evaluation and prognosis.
