MAP液保存的冰冻解冻去甘油红细胞可保存天数的研究 被引量:1

摘要 目的研究悬浮于MAP液中的冰冻解冻去甘油红细胞最多是否可保存7 d。方法将11份2 U悬浮红细胞制品制备成冰冻解冻去甘油红细胞,冻存1年,解冻去甘油后悬浮于MAP液中,于0 h、3 d、5 d和7 d取样检测钾离子浓度(K+)、ATP浓度、2,3-DPG浓度、游离血红蛋白含量(FHb)和溶血率。结果 ATP浓度随着保存时间的延长略有下降,保存7 d的ATP浓度为(5.36±0.91)μmol/g Hb,ATP回收率仍高达94%。2,3-DPG浓度下降速度较ATP快,但保存7 d的红细胞仍然存在2,3-DPG,浓度为(0.62±0.29)μmol/g Hb。保存7 d的FHb为(0.30±0.12)g/L,符合GB18469-2012对冰冻解冻去甘油红细胞FHb的规定(≤1 g/L)。溶血率由解冻后的(0.05±0.01)%上升至7 d的(0.11±0.04)%,符合国标保存期末溶血率≤0.8%的要求。解冻后保存3 d的K+上升至(5.70±1.09)mmol/L,已超出人体正常K+参考范围上限(5.5 mmol/L)。结论 MAP液保存冰冻解冻去甘油红细胞可以保存7 d。但对于高钾血症贫血患者这部分特殊人群,若以不超过人体正常K+参考范围上限为判断依据,则MAP液保存的冰冻解冻去甘油红细胞,最好不要超过24 h。 Objective To study the preservation time of deglycerolized red blood cells( RBCs) in MAP additive solution. Methods Eleven bags of suspended RBCs( 2 U) were glycerolized and frozen for one year. Frozen RBCs were then deglycerolized and preserved in MAP. The concentration of K^+,ATP,2,3-DPG,and FHb and the percentage of hemolysis were analyzed at 0d,3d,5d and 7d. Results The concentration of ATP decreased slightly as the post-thawed preservation time prolonged. The concentration of ATP at 7d was( 5. 36 ± 0. 91) μmol / g Hb and the recovery rate of ATP was still as high as94%. The concentration of 2,3-DPG dropped faster than that of ATP,but traces of 2,3-DPG still existed at 7 d( 0. 62 ± 0. 29μmol /g Hb). The concentration of FHb at 7 d was( 0. 30 ± 0. 12) g /L,which met GB18469-2012 in deglycerolized RBCs( ≤1 g/L). The percentage of hemolysis increased from( 0. 05 ± 0. 01) %( 0 d) to( 0. 11 ± 0. 04) %( 7 d),which accorded with the national hemolysis requirement at the end of storage period( ≤0. 8%). The concentration of K+at 3 d increased to( 5. 70 ± 1. 09) mmol / L,which was already higher than the upper limit of K+in normal human bodies( 5. 5mmol / L). Conclusion Deglycerolized RBCs can be preserved in MAP for 7d. However,for hyperkalemic patients with anemia,the preservation time of deglycerolized RBCs in MAP should not exceed 24 hours,in view of that K+in deglycerolized RBCs is not higher than the upper limit of K+in normal human bodies.
