摘要
目的:系统评价冠心病与ABO血型分布的关系。方法计算机检索Cochrane Library、PubMed、万方数据库、中国学术期刊数据库(CNKI)、中国生物医学文献数据库(CBM)至2014年1月31日,收集冠心病与ABO血型关系的病例对照研究及队列研究,按纳入和排除标准由2名评价者独立选择试验、提取资料,交叉核对并进行方法学质量评估后,使用RevMan5.2软件进行Meta分析。结果纳入17项研究,冠心病患者15988例。Meta分析结果显示:①与对照组比较,冠心病患者中A血型的比例升高(OR=1.27,95%CI=1.09-1.48,P〈0.01),O血型的比例降低(OR=0.79,95%CI=0.68-0.91,P〈0.01),B血型(OR=1.04,95%CI=0.93-1.16,P〉0.05)与AB血型(OR=0.88,95%CI=0.70-1.09,P〉0.05)比较差异无统计学意义;②与对照组比较,心肌梗死(MI)组O血型的比例降低(OR=0.63,95%CI=0.45-0.86,P〈0.01)。两组A、B、AB血型MI分布比较差异无统计学意义。结论A血型人群患冠心病的风险增加,而O血型则为冠心病与MI发病的保护因子。B血型及AB血型与冠心病及MI的发病均无明显相关性。
Objective To objectively evaluate the correlation between coronary heart disease(CHD)and ABO blood groups.Methods Such databases as Cochrane Library,PubMed,Wanfang,CNKI,and CBM were searched from their establishment to January 31,2014 for collecting the case-control study and cohort study about CHD and ABO blood group systems. After study selection,assessment and data extraction were conducted by two reviewers independently,meta-analysis was performed using the RevMan 5.2 software and Stata 12.0 software.Results Seventeen studies involving 1 5 988 patients were included.The results of meta-analysis showed that:① compared with controls,CHD patients had higher proportion in blood group A(OR = 1.27,95% CI = 1.09-1.48,P 〈0.01 ),less proportion in group O(OR =0.79,95% CI =0.68-0.91,P 〈0.01),and the CHD proportion showed no significant difference between blood group B(OR =1.04,95% CI =0.93-1.1 6,P 〉0.05)and AB(OR =0.88,95% CI =0.70-1.09,P 〉0.05);②compared with healthy controls,myocardial infarction(MI)patients had less proportion in group O (OR =0.63,95% CI =0.45-0.86,P 〈0.01),and the MI proportion showed no significant difference among group A,group B and AB.Conclusion Blood group A population has a higher risk for CHD,whereas group O was a protective factor for CHD and MI,there were no significant association between group B/AB and CHD/MI.
