摘要
目的了解强直性脊柱炎(AS)不同临床分期患者外周血淋巴细胞人类白细胞抗原B27(HLA-B27)表达量与疾病临床分期之间的关系。方法采用三色流式细胞学检测技术分析88例不同阶段AS患者及132例健康人的外周血淋巴细胞HLA-B27表达率及表达强度(MNX)。结果 AS进展期患者外周血淋巴细胞HLA-B27表达率的中位数、P25、P75分别是95.34%、86.60%、97.39%,MNX的中位数、P25、P75分别为8.70、5.20、10.80;强直期患者外周血淋巴细胞HLA-B27表达率的中位数、P25、P75分别是94.51%、90.40%、97.83%,MNX的中位数、P25、P75分别为8.40、6.05、15.95。健康对照组的外周血淋巴细胞HLA-B27表达率的中位数、P25、P75分别是3.69%、1.03%、11.13%,MNX的中位数、P25、P75分别为1.50、1.30、1.90。Mann-Whitney U检验显示,AS患者进展期与强直期的外周血淋巴细胞HLA-B27表达率及MNX均差异无统计学意义(P>0.05);进展期、强直期与健康对照组的外周血淋巴细胞HLA-B27表达率及MNX均差异有统计学意义(P<0.05)。结论外周血淋巴细胞HLA-B27的表达率及MNX适合于AS诊断,对AS病情判断无辅助意义。
Objective To study the relationship between the expression amount of peripheral blood lymphocyte HLA-B27 with the clinical stages in the patients with different clinical stages of ankylosing spondylitis（AS）.Methods The expression rate and the expression intensity（MNX）of peripheral blood lymphocyte HLA-B27 were detected in 88 cases of different stages of AS and 132 healthy individuals by the three-color flow cytometry and the results were analyzed statistically.Results The median number,P25 and P75of peripheral blood lymphocyte HLAB27 expression rate in the patients with progressive stage of AS were 95.34%,86.60%and 97.39%respectively,the median number,P25 and P75of MNX were 8.70,5.20 and 10.80respectively;the median number,P25 and P75of peripheral blood lymphocyte HLA-B27 expression rate in the rigidity stage were 94.51%,90.40% and97.83%respectively,the median number,P25 and P75of MNXwere 8.40,6.05 and 15.95 respectively.The median number,P25,P75 of peripheral blood lymphocyte HLA-B27 expression rate in the healthy control group were3.69%,1.03%and 11.13%,respectively,the median number,P25 and P75of MNX were 1.50,1.30 and 1.90 respectively.The Mann-Whitney U test showed that the expression rate and MNX of peripheral blood lymphocyte HLA-B27 no statistically significant differences between the progressive stage and the rigidity stage in AS patients（P〉0.05）;the differences in the expression rate and MNX of peripheral blood lymphocyte HLA-B27 among the progressive stage,rigidity stage and the healthy control group had statistical significance.Conclusion The expression rate and MNX of peripheral blood lymphocyte HLA-B27 are suitable for the diagnosis of AS and have no auxiliary significance in the judgment of AS condition.
