摘要
目的：比较不同给药方法预防及减少舒芬太尼诱发咳嗽反射的效果。方法采用随机分组方法,将入选患者分为6组,每组20~22例。 A组：利多卡因1 mg/kg静注；B组：地塞米松10 mg静注；C组：氟比洛芬酯1 mg/kg静注；D组：右美托咪定1μg/kg,10 min泵注；E组：舒芬太尼0．4μg/kg,30 s推注；F组：舒芬太尼正常推注（干预措施为推注等剂量生理盐水）。除E组外,其他组均给予舒芬太尼0．4μg/kg推注,2 s内推注完毕。观察舒芬太尼推注1 min后患者咳嗽发生例数及咳嗽严重程度。结果各组咳嗽反射的发生率差异虽无统计学意义（P〉0．05）,但从趋势上来看,E组（舒芬太尼缓慢推注）发生率（9．1%）略低于其他各组（28．6%~31．8%）。 E组（舒芬太尼缓慢推注）的咳嗽严重程度轻于其他各组,差异有统计学意义（ P〈0．05）。结论舒芬太尼缓慢静注能够明显降低麻醉诱导期舒芬太尼引起的咳嗽反应发生率,并减轻其引起咳嗽反射的严重程度。
Objective To compare the effects of different administration methods on preventing and reducing cough reflex induced by sufentanil,and then find out the most effective and the best way to inhibit sufentanil cough re-flex. Methods The patients were allocated to 6 groups randomly,average 20 ~22 cases in each group. Group A：the group of lidocaine 1 mg/kg;Group B：the group of dexamethasone 10 mg;Group C：the group of flurbiprofen axetil 1 mg/kg;Group D：the group of dexmedetomidine 1 μg/kg, 10 min pumping-in;Group E：the group of sufentanil 0. 4 μg/kg,30s slow-injection;Group F：blank control group：inject isodose normal saline. Then Sufentanil was injected 0. 4 μg/kg and finished in 2 s except Group E（sufentanil 0. 4 μg/kg in 30 s）. After 1 min of sufentanil injection, cough times,occurrence time and serious degree of patients were observed. Results There was no significant differ-ence in cough incidence rate due to the small sample size（P〉0. 05）. But in the tendency,group E（sufentanil slow in-fusion）with rate of 9. 1%was slightly lower than those of other groups（28. 6% ~ 31. 8%）. Cough severity had statisti-cal differences（P〈0. 05）,and the cough severity of group E（slow infusion of sufentanil）was statistically significant compared with the other groups. Conclusion Slow intravenous injection of sufentanil can inhibit the cough reflex in the induction of anesthesia. In the statistical point,it shows significant degree in suppressing cough,and also shows a tendency in inhibiting the occurrence of cough.
