摘要
目的:研究胸腹腔镜联合Ivor Lewis手术与开放手术对中下段食管癌患者血清学指标的影响。方法:将在本院接受Ivor Lewis手术的80例中下段食管癌患者纳入研究并随机分为两组,观察组接受胸腹腔镜联合Ivor Lewis手术、对照组接受开放Ivor Lewis手术,检测两组的血清过氧化指标、肿瘤标志物以及肿瘤恶性生物学行为指标。结果:(1)过氧化指标:观察组的丙二醛(MDA)、晚期氧化蛋白产物(AOPP)含量低于对照组,谷胱甘肽过氧化物酶(GSH-Px)含量高于对照组,差异有统计学意义(P<0.05);(2)肿瘤标志物:观察组的转录因子YY1、可溶型MHC-I类链相关蛋白A(sMICA)以及细胞角蛋白19片段抗原21-1(Cyfra21-1)含量均低于对照组,差异有统计学意义(P<0.05);(3)肿瘤恶性生物学行为指标:观察组血清中血管内皮生长因子(VEGF)、CD44v、基质金属蛋白酶-9(MMP-9)含量低于对照组,差异有统计学意义(P<0.05)。结论:胸腹腔镜联合Ivor Lewis手术有助于缓解过氧化反应、减少肿瘤组织受到的刺激、控制肿瘤恶性生物学行为,是治疗中下段食管癌的理想方法。
Objective：To study the effects of thoracic laparoscopic Ivor Lewis operation and open operation on serum indexes of esophageal cancer patients.Methods：80esophageal cancer patients received IvorLewis operation in our hospital were enrolled and randomly divided into two groups.Observation group received thoracic laparoscopic Ivor Lewis operation,control group received open Ivor Lewis operation.Then peroxidation index,tumor markers and tumor malignant biological behavior index were detected.Results：（1）Peroxidation index：MDA and AOPP contents of observation group were lower than those of control group;GSH-Px content was higher than that of control group;（2）tumor markers：YY1,sMICA and Cyfra21-1contents of observation group were lower than those of control group;（3）Malignant tumor biological behavior index：VEGF,CD44 v,MMP9contents of observation group were lower than those of control group.Conclusion：Thoracic laparoscopic Ivor Lewis operation is helpful to alleviate peroxidation reaction,reduce tumor tissue stimulation,control tumor malignant biological behavior;it’s an ideal method in treating esophageal cancer.
