锁定钢板和交锁髓内钉治疗肱骨干骨折临床对照研究 被引量:2

摘要 目的比较切开复位锁定钢板内固定与闭合复位逆行交锁髓内钉内固定治疗肱骨干骨折的效果。方法 42例肱骨干骨折患者,采用随机数字表分为两组,锁定钢板组22例,采用锁定钢板内固定治疗;髓内钉组20例,采用逆行髓内钉内固定治疗,比较两组手术时间、术中出血量、住院时间、临床疗效及术后并发症。结果两组患者术后随访12~36个月,平均15个月。髓内钉组手术时间、术中出血量明显少于锁定钢板组(P〈0.05),两组住院时间差异无统计学意义(P〉0.05)。髓内钉组骨延迟愈合率为35.0%(7/20),明显高于锁定钢板组的9.1%(2/22)(P〈0.05)。术后锁定钢板组肩关节功能优17例,良2例,差3例,髓内钉组优16例,良1例,差3例,两组肩关节功能疗效比较,差异无统计学意义(P〉0.05)。结论锁定钢板及交锁髓内钉固定均为治疗肱骨干骨折有效的手术方法,交锁髓内钉固定的手术时间短,术中出血量少,但骨延迟愈合率较高。
