造影剂剂量对健康成年人肝脏超声造影定量分析的影响 被引量:3

The effect of the contrast agent dose of quantitative contrast enhanced ultrasound in healthy adults liver

摘要 目的探讨超声造影剂剂量对健康成年人肝脏造影效果的影响。方法选取首都医科大学附属北京友谊医院健康志愿者20名,分别在4个剂量水平(0.5、1.2、1.8、2.4 ml)团注超声造影剂Sono Vue(声诺维),采用QLAB 9.0软件自动描记时间-强度曲线,定量分析每一剂量在相同监测条件下的常用血流灌注参数。声学定量指标包括开始显影时间(AT)、达峰时间(TTP)、上升时间(Δt)、本底强度(Ib)、峰值强度(Ip)、增强强度(ΔI)及ΔI/Δt。结果当造影剂剂量为0.5-2.4 ml范围时,Ip及ΔI值随造影剂剂量增加而增大。剂量为0.5 ml与1.2 ml时,Ip分别为(26.7±3.7)d B和(33.5±4.3)d B,差异有统计学意义(t=2.84,P〈0.05);ΔI分别为(13.1±1.2)d B和(16.8±2.7)d B,差异亦有统计学意义(t=2.90,P〈0.05)。当造影剂剂量为0.5-2.4 ml范围时,TTP值分别为(31.2±3.6)、(28.4±5.2)、(27.7±4.1)和(24.5±4.7)s,随剂量增大而缩短(F=8.40,P=0.000)。0.5 ml与1.2 ml时比较、1.2 ml与1.8 ml时比较、1.8 ml与2.4 ml比较,TTP的差异均有统计学意义(t=2.65、2.25、3.18,P均〈0.05)。Δt和ΔI/Δt值随剂量增加无明显改变,不同剂量间比较差异无统计学意义。结论声学造影剂剂量影响肝造影增强各参数,峰值强度与应用的造影剂剂量有关。时间参数具有较好的稳定性,具有较高的临床使用价值。 Objective To investigate the influence of contrast agent(CA) dose on contrast effect in healthy adults liver.Methods Sono Vue was applied in four different doses(0.5 ml,1.2 ml,1.8 ml,2.4 ml) respectively among 20 volunteers(Beijng Friendship Hospital,Capital Medical University).Timeintensity curve(TIC) and quantitative parameters were acquired by QLAB 9.0 softversion to analyze the relationship between CA dose and enhancement pattern.The perfusion parameters contained arrival time(AT),time to peak intensity(TTP),Δt(Δt=TTP-AT) and ΔI/Δt,basic intensity(Ib),peak intensity(Ip),ΔI(ΔI =Ip-Ib).Results Along with an increase of the Sono Vue doses from 0.5 ml to 2.4 ml,the value of Ip and ΔI were increased.When the Sono Vue dose was 0.5 ml and 1.2 ml,the value of Ip were(26.7±3.7) d B and(33.5±4.3) d B,the differences of Ip were statistically significant(t=2.84,P〈0.05);the value of ΔI were(13.1±1.2) d B and(16.8±2.7) d B,the differences of ΔI were statistically significant(t=2.90,P〈0.05).When compared with the dose of 1.2 ml,1.8 ml,the dose of 1.8 ml and 2.4 ml,no statistical significance was obtained between the two groups.From 0.5 to 2.4 ml,the value of TTP((31.2±3.6)s,(28.4±5.2)s,(27.7±4.1)s,and(24.5±4.7)s) were shortened with the increase of dose.When compared with the dose of 0.5 ml and 1.2 ml,differences of TTP were statistically significant(t=2.65,P〈0.05).Comparing the dose of 1.2 ml with 1.8 ml,differences of TTP were statistically significant(t=2.25,P〈0.05).Comparing the dose of 1.8 ml with 2.4 ml,differences of TTP were statistically significant(t=3.18,P 0.05).No significant differences were observed between the UCA dose and the value of Δt and ΔI/Δt.Conclusions Different doses of CA have great effect on the quantitative parameters,especially the peak intensity.Time parameters show better stablity and clinical value.
