摘要
目的观察DEX鞘内注射对剖宫产大鼠神经行为学及脾脏c-fos表达的影响。方法 80只孕18d SD大鼠,随机分成5组,每组16只,CON组只进行麻醉,不进行剖宫产和鞘内注射;MOD组麻醉后进行剖宫产,进行鞘内注20μ生理盐水;DEX1,DEX2,DEX3组分别在麻醉后剖宫产大鼠,鞘内注射10μg/kg、30μg/kg、50μg/kg DEX,每组中的8只观察术前及术后12h、24h、48h右后肢PWT和PWL的变化。每组余下8只术后6h颈椎脱臼致死,取脾脏进行免疫组化检测c-fos表达。结果MOD组术后PWT、PWL较CON组明显降低(P<0.05),逐渐升高(P<0.05),DEX1、DEX2及DEX3组术后PWT、PWL较MOD组明显降低,和DEX注射剂量呈相关性,随着时间推移逐渐升高(P<0.05)。DEX1、DEX2、DEX3之间也有明显差异(P<0.05)。MOD组脾脏c-fos表达较CON组明显升高(P<0.05),鞘内注射DEX后较MOD组明显降低(P<0.05),并呈剂量相关。结论DEX鞘内注射能明显改善剖宫产大鼠疼痛敏化,脾脏c-fos表达降低,呈剂量相关性。
Objective To observe the affects of dexmedetomidine intrathecally on neurobehavioral and expression of c-fos in spleen in cesarean section rats. Methods A total of 80 experimental rats pregnant 18 days were randomly divided into five groups with 16 rats in each group. No intervention except anesthesia in CON group,The CON plus cesarean section and 0.9% NS 20μl intrathecally in MOD group. Dexmedetomidine administered intrathecally before cesarean section to rats after anesthesia in DEX1 , DEX2,DEX3 groups,intrathecally administered DEX with 10μg/kg,30μg/kg,50μg/kg respectively. Observe the affects of DEX on PWT and PWL on right hind leg in eight rats of each group at preoperative and postoperative 12h,24h,48h. the remained eight rats each group were killed by cervical dislocation after 6h postoperative ,and take spleen for immunohistochemistry to detect c-fos expression. Results The PWT and PWL value were significantly reduced postoperative in MOD group (P〈0.05),and increased gradually after cesarean section (P〈0.05),controlled with MOD group,the PWT and PWL value in DEX1,DEX2 and DEX3 groups were decreased obviously,administration of DEX dose and PWT,PWL appeared related,increased gradually after cesarean section. The level of c-fos in spleen in MOD group was obviously higher than CON group,and the levels in DEX1,DEX2,DEX3 group were significantly lower than MOD group (P〈0.05),and the level of c-fos in spleen was obviously different between DEX1 group and DEX2,DEX3 groups (P〈0.05). and was related with dose of DEX. Conclusion Administered dexmedetomidine can obviously improve pain sensitization in the cesarean section rats,reduced c-fos expression in spleen,and is related with dose of DEX.
