摘要
目的:研究初发2型糖尿病(T2DM)患者血清脂联素(APN)、瘦素(LP)、白介素-18(IL-18)的变化与糖脂代谢的关系。方法:检测并比较正常体重(A组,10例)及超重(B组,10例)非糖尿病自愿者,正常体重(C组,32例)及超重(D组,35例)2型糖尿病患者的体重指数(BMI)、空腹血糖(FPG)、空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR),APN、IL-18及LP。结果:B组及D组LP、IL-18高于A组及C组(P<0.05),且C组IL-18高于B组(P<0.05),APN在A、B、C、D 4组中逐渐下降,组间比较,差异有统计学意义(P<0.05),IL-18与BMI、HOMA-IR正相关,与APN负相关(P<0.05);APN与BMI、LP、HOMA-IR负相关(P<0.05);LP与BMI正相关(P<0.05)。结论:低APN和高IL-18血症可能是糖尿病的危险因素,其代谢紊乱与胰岛素抵抗密切相关,而LP可能只与肥胖相关。
Objective: To investigate the correlation between levels of adiponectin (APN), leptin (LP), change of interleukin-18 (IL-18) of TzDM patients and the glucose metabolism. Methods: 10 non-diabetic cases with (Group A), 10 non-diabetic cases with normal weight overweight (Group B), 32 cases of type 2 diabetes mellitus with normal we betes mellitus with over weight (Group D) were include lg d ht (Group C), and 35 cases of type 2 dia- in the study respectively. The correlation of BMI, adiponectin, IL-18, fasting plasma glucose (FPG), fasting insulin (FINS), insulin resist- ance of homeostasis model assessment (HOMA-IR), APN, IL-18 and LP were detected among all patients respectively. Results: LP and IL-18 levels in Group B and D were significantly higher than those in Group A and C respectively (P〈0.05), IL-18 levels in C group was also higher than that in B group, and APN levels gradually decreasedin four groups (P〈0. 05). Correlation analysis showed that IL-18 was positively correlated with BMI, HOMA-IR, but negatively correlated with APN (P〈0.05), and APN was negatively LP was positively correlated with BMI in Low APN and high IL-18, LP may be one ic disorders are closely related with insulin correlated with BMI, LP, HOMA-IR (P〈0.05), while patients with type 2 diabetes (P〈0.05). Conclusion: of the factor in pathogenesis of diabetes, whose metabol resistance.
