实时三维子宫输卵管超声造影评价输卵管通畅性及其影响因素分析 被引量:26

摘要 目的采用经阴道实时三维超声造影(RT 3D-HyCoSy)评价输卵管通畅性,并对影响诊断准确性的相关因素进行探讨。方法对88例共169条输卵管行RT 3D-HyCoSy检查,观察RT 3D-HyCoSy的显影特征,评估输卵管通畅性,并与腹腔镜下美兰通液结果进行对照。按照子宫位置,有无造影剂肌层逆流,附件区有无包块,分组比较诊断符合率。结果 169条输卵管,RT 3D-HyCoSy诊断输卵管阻塞敏感性82%、特异性93.2%,诊断符合率89.9%。前位或后位子宫组诊断符合率(94.4%)高于中位子宫组(64%);无造影剂肌层逆流组诊断符合率(94.2%)高于有肌层逆流组(79.5%);附件区无包块组诊断符合率(91.7%)高于有包块组(66.7%);以上差异均有统计学意义。结论经阴道实时三维子宫输卵管超声造影评价输卵管通畅性结果可靠;中位子宫,造影剂肌层逆流,附件区包块是影响诊断准确性的重要因素。 Objective To assess the fallopian tube patency with transvaginal real-time three Dimensional hysterosalpingo-contrast sonography (RT-3D-HyCoSy) and to analyse of influence factors. Methods 88 infertile women (a total of 169 tubes) underwent transvaginal RT-3D-HyCoSy. All of them accepted the laparoscopically salpingoraphy to evaluate the fallopian tube patency. The accuracy was compared between the groups divided by the different location of the uterus, the sign of contrast agent reflux, the mass of adnexa and the doctor's experience. Results Compared with laparoscopy sensitivity, specificity and accuracy in diagnosing tubal patency by using RT-3D-HyCoSy are 82%, 93.2%, 89.9% respectively. The accuracy in a group with anteverted/retroverted uterus (94.4%) is higher than another group with midpostion uterus (64%). The accuracy in a group with reflux sign (94.2%) is higher than another one (79.5%). The accuracy in a group with mass of adnexa (91.7%) is higher than another one (66.7%). There is statistical difference. Conclusion Transvaginal RT-3D-HyCoSy could provide a novel and relible approach to assess fallopian tube pantency. The postion of uterus, the sign of contrast agent reflux and the mass of adnexa are importing influence factors of RT-3D-HyCoSy.
