摘要
目的比较排便训练技术和聚乙二醇药物治疗产后功能性便秘患者的应用效果,探讨肛管直肠功能和临床症状的改善情况。方法选取产后功能性便秘患者81例,根据自愿原则分为观察组(n=42)和对照组(n=39)。对照组遵医嘱给予口服聚乙二醇,观察组根据患者功能性便秘分型情况选择排便训练方案。观察比较两组患者干预前和干预结束6个月末肛管直肠压力、直肠感觉功能和临床症状。结果干预结束6个月末,观察组力排时直肠压力、排便指数高于对照组(P<0.01),力排时肛管剩余压力低于对照组(P<0.01);观察组直肠初始感觉容量、初始便意容量、排便窘迫容量、最大耐受容量均低于对照组(P<0.01);患者对便秘症状改善情况的满意度评分、临床疗效高于对照组(P<0.01)。结论与口服聚乙二醇相比,排便训练技术能更有效地修复产后功能性便秘患者的盆底肌、腹肌、肛管直肠功能,改善肛门括约肌的协调性,修复直肠感觉功能,并缓解其临床症状。
Objective To evaluate the clinical effects of bowel training and oral polyethylene glycol on postpartum patients with functional constipation(FC). Methods Eighty-one postpartum FC patients were divided into test group(n =42) and control group(n=39) voluntarily. Patients in control group took polyethylene glycol and those in test group received bowel training according to the classification of constipation. The anorectal motility, anorectal sensory function and clinical symptoms of the two groups were evaluated before and 6 months after the treatment. Results Six months after the interventions, the rectal squeeze pressure and defecation index of test group were significantly higher than those of control group( P0.01); the residual anal pressure of test group was significantly lower than that of control group(P 0.01). Meanwhile, the first sensation of distention, sensation of defecation,sensation of urgency and maximum tolerable volume of test group were significantly lower than those of control group( P0.01). Good clinical efficacy was achieved and patients were more satisfied with the alleviation of constipation in test group( P 0.01).Conclusion Compared with the oral polyethylene glycol, bowel training could effectively reconstruct the functions of pelvic floor muscles, abdominal muscles and anorectal motility, improve the coordination of anal sphincter, reduce the threshold of rectal sensation, and finally relieve the clinical symptoms of postpartum FC patients.
