摘要
目的:观察舒适护理联合地佐辛在下肢骨折患者行坐位腰硬联合麻醉穿刺中的应用效果。方法:100例下肢骨折患者随机均分为对照组(C组)、舒适护理组(N组)、地佐辛组(D组)和联合组(ND组)4组,每组25例。C、D组予常规护理,N、ND组予舒适护理,D、ND组患者入室后静脉注射地佐辛0.1 mg·kg-1。术前1 d和麻醉前测定患者焦虑值,观察麻醉穿刺时间、穿刺中患肢疼痛VAS评分和Sp O2水平以及患者满意度。结果:4组患者Sp O2比较差异无统计学意义(P>0.05);与C组比较,其余3组麻醉前SAS评分、穿刺时间、穿刺中患肢VAS评分降低(P<0.05),患者满意度升高(P<0.05);与N组和D组比较,ND组患肢VAS评分降低(P<0.05),患者满意度升高(P<0.05)。结论:舒适护理联合地佐辛可安全有效缓解患者麻醉前焦虑,缩短麻醉穿刺时间,减轻坐位穿刺时的患肢疼痛,提高患者满意度。
Objective： To study the effects of comfortable nursing and dezocine in patients with fracture of lower limb undergoing spinal-epidural anesthesia puncture with sitting position. Methods： One hundred patients with lower limb fracture were randomized into four groups（ n = 25） ： control group（ group C）,comfortable nursing group（ group N）,dezocine group（ group D） and combined group（ group ND）. The group C and D received regular nursing. Group N and NE were given comfortable nursing. In group D and NE,dezocine 0. 1 mg · kg- 1was injected after entering operating room. The self-rating anxiety scale（ SAS） score was measured 1d before operation and before anesthesia. Time of puncture,the visual analogue scale（ VAS） of limb pain,level of Sp O2 in puncture process and satisfaction of patients were recorded also. Results： There was no significant difference of the level of Sp O2 between four groups（ P〉0. 05）. Compared with group C,the score of SAS before anesthesia and time of puncture and the VAS of limb pain were significantly decreased and the satisfaction of patients was increased in the other three groups（ P〈0. 05）. Compared with group N and D,the VAS of limb pain was significantly decreased and the satisfaction of patients was increased in group ND（ P〈0. 05）. Conclusion： Comfortable nursing combined with dezocine can obviously relieve anxiety before anesthesia,meanwhile decreasing the time of puncture,easing the lower limb pain and improving the satisfaction of patients.
