摘要
目的评价国内Vitapex用于根尖诱导成形术的有效性。方法计算机检索维普资讯中文科技期刊数据库(VIP)、清华同方数据库中国期刊全文数据库(CNKI)、万方数据库等。检索时间均为建库至2012年2月。收集Vitapex用于根尖诱导成形术的随机对照试验(RCT)。按照Cochrane Handbook 5.0推荐的"偏倚风险评估"工具对纳入研究进行方法学质量评估,利用RevMan 5.0软件对同质研究进行Meta分析。结果共纳入11个RCT研究,1 431例患牙。Meta分析结果显示,对于临床根尖诱导成形术总有效率,Vitapex治疗组优于氢氧化钙(CH)治疗组,与抗生素、MTA疗效相当。对于根端喇叭口型、管壁平行型患牙,Vitapex、氢氧化钙(CH)、抗生素三者疗效相当;对于管壁内聚型、牙髓炎、牙髓坏死、根尖周炎患牙,Vitapex行根尖诱导成形术的疗效明显优于CH。结论Vitapex是一种较好的根尖诱导材料,由于纳入研究的文献方法学质量限制,有待更多高质量文献验证。
Objective To access the effect of Vitapex for apexification.Methods The retrieval was carried out in the databases including VP,CNKI and Wanfang from the beginning of their establishment to February 2012.Collecting random controlled trials about apexification with Vitapex.In accordance with bias risk assessment tool"recommended by Cochrane Handbook 5to rigorously evaluate the methodological quality of included studies,do Meta analysis using RevMan 5.0software for homogeneous studies.Results Eleven studies including 1431 teeth were included.Meta analysis showd that Vitapex was more effective in clinical efficacy rate compared with calcium hydroxide for apexification,but equivalent in efficacy compared with antibiotics and MTA.The effect of apexification with either Vitapex,calcium hydroxide and antibiotics share the same effect on the tooth with parallel root canal wall and horn shaped root canal orifice.However,the effect of apexification with Vitapex on the tooth with cohesive root canal wall,periapical periodontitis,pulpitis and pulp necrosis is superior to that of their counterpart with calcium hydroxide.Conclusion Vitapex paste is an ideal filling material in apexification.However,the potential benefits need to be confirmed in rigorous clinical trials due to poor methodological quality in the majority of included studies.
