血清同型半胱氨酸水平与青年急性脑梗死严重程度及CISS分型的关系 被引量:16

Relationship between Serum Hcy Levels and Stroke Severity and Subclassification in Young Patients with Acute Cerebral Infarction

摘要 目的探讨青年急性脑梗死(ACI)患者血清同型半胱氨酸(Hcy)与梗死严重程度及中国缺血性卒中亚型(CISS)分型之间的关系。方法应用酶联免疫吸附法检测332例年龄18岁~45岁急性脑梗死患者血清Hcy水平,按CISS分型、神经功能缺失程度(NIHSS评分)进行分组,将各亚组之间以及与179例年龄18岁~45岁健康对照者进行比较。结果病例组血清Hcy水平显著高于对照组[(19.07±10.83)μmol/L vs(7.48±3.68)μmol/L,t=16.36,P〈0.01];轻、中、重型患者Hcy水平分别为(16.26±6.37)μmol/L、(20.81±6.21)μmol/L、(24.88.46±6.49)μmol/L,组间差异有统计学意义(P〈0.05);不同病因和发病机制患者血清Hcy水平差异无统计学意义(P〉0.05)。结论血清Hcy水平在青年脑梗死急性期升高,与脑梗死发生密切相关,可作为判断ACI病情严重程度的检测指标,但在不同病因和发病机制患者血清Hcy水平无统计学意义。 Objective To investigate the relationship betwe en serum homocysteine(Hcy)levels and stroke severity and subclassification in young patients with acute cerebral infarction.Methods The serum Hcy levels in332 patients of 18to45 years old with acute cerebral infarction(ACI)were measured by enzyme-linked immtmosorbentassay(ELISA),and they were grouped according to CISS subclassification and National Institutesof Health Stroke Scale(NIHSS).The patients between all the subclassification and 179 healthy controls of 18 to 45 years old were compared.Results The serum Hcy levels in patient group were significantly higher than those in control group[(19.07±10.83)μmol/L vs(7.48±3.68)]μmol/L.The serum Hcy levels in light,medium,heavy stroke group were(16.26±6.37)μmol/L,(20.81±6.21)μmol/L,(24.88.46±6.49)μmol/L,and the different of groups were statistically significant.No futher significantly difference was foud among different subclassification.Conclusion The serum Hcy levels increased in young patients with ACI,it closely related with the occurrence of cerebral infarction,and could be used to be a indicator to determine the severity of ACI.No futher significantly difference was foud among different subclassification.
