淋巴结转移率与乳腺癌预后关系的研究 被引量:10

摘要 目的分析淋巴结转移率(转移淋巴结数/切除淋巴结总数)在乳腺癌患者预后的作用。方法收集2001年1月至2008年12月收治的浸润性乳腺癌患者1 192例,通过Kaplan-meier生存曲线及Cox模型分析淋巴结转移率在评估乳腺癌患者预后的作用。结果单因素分析结果显示T分期、N分期、淋巴结转移率和体重指数是影响乳腺癌患者无瘤生存时间和总生存时间的危险因素,雌孕激素受体阳性是保护性因素。多因素分析结果显示T分期、淋巴结转移率和体重指数仍然是危险因素,而N分期失去了统计学意义,雌孕激素受体阳性是保护性因素。结论与N分期相比,淋巴结转移率能更好的评价乳腺癌患者术后的复发风险和总生存时间。 ObjectiveTo evaluate the prognostic impact of the lymph node metastasis rate(LNR;i.e.,the ratio of metastasized to dissected lymphnodes)on the recurrence and survival of breast cancer patients.MethodsData from a cohort of 1 192 invasive breast cancer patients diagnosed inThe First Hospital,China Medical University between 2001 and 2008 were analyzed for disease-free survival(DFS)and overall survival(OS)toassess the prognostic impact of LNR in breast cancer patients.ResultsIn univariate analyses,T stage,N stage,LNR and BMI were the risk factorssignificantly associated with DFS and OS of breast cancer patients,while positive estrogen and progesterone receptor was a protective factor. A multi-variate analysis using the Cox proportion hazards model found that T stage,LNR and BMI were still the risk factors,while N stage lost statistical sig-nificance,and positive estrogen and progesterone receptor was a protective factor.ConclusionThe LNR appears to be a better prognostic factor forDFS and OS,compared to N stage.
