摘要
目的探讨腹腔镜与开腹Miles术治疗直肠癌的临床疗效。方法选择2009年1月至2011年1月间收治的直肠癌患者120例,根据手术方法将其分为腹腔镜组(54例)和开腹组(66例)。腹腔镜组采取腹腔镜Miles直肠癌根治术,开腹组采取传统开腹Miles直肠癌根治术。结果腹腔镜组术中出血量显著低于开腹组(P<0.05),但手术时间长于开腹组(P<0.05);两组淋巴结清扫数差异无统计学意义(P>0.05)。腹腔镜组术后胃肠功能恢复时间、肛门排气时间、术后下床时间、进食流质食物时间及住院时间显著低于开腹组(P<0.05)。两组患者术后并发症发生率差异无统计学意义(P>0.05)。两组患者术后1、3、5年的生存率差异无统计学意义(P<0.05)。结论腹腔镜与开腹Miles术治疗直肠癌的远期临床疗效相同,具有出血少、创伤小、术后恢复速度快、住院时间短等优势,适于临床全面推广。
Objective To compare long-term and short-term effect of laparoscopic and open Miles in the treatment of rectal cancer. Methods Selected 120 cases of patients with rectal cancer from January2009 to January 2011 in Ding＇an County People＇s Hospital of Hainan Province,according to the operation were divided into 54 cases of laparoscopic group and 66 cases of laparotomy group. Laparoscopic Miles radical resection of rectal cancer received laparoscopic radical surgery,laparotomy group take the traditional open Miles in rectal cancer. Results The amount of bleeding of laparoscopic group was significantly lower than that of the control group（ P〈0. 05）,but the operation time is longer than the open surgery group（ P〈0. 05）; no significant difference between the two groups of lymph dissection contrast（ P〉0. 05）.Time,anus exhaust time,postoperative recovery of gastrointestinal function in the laparoscopic group after bed time,postoperative eating liquid food and hospitalization time was significantly lower than that of the control group（ P〈0. 05）. The postoperative complications of two groups had no significant difference compared（ P〉0. 05）. The two groups after 1-year,3-year,5-year survival rate compared no significant difference（ P〉0. 05）. Conclusions The laparoscopic and the same as the long-term clinical efficacy of open Miles for rectal cancer,at the same time with less bleeding,less trauma,faster postoperative recovery,short hospitalization time and other advantages,and is suitable for clinical comprehensive promotion.
