摘要
目的探讨连续性静一静脉血液透析滤过治疗慢性心力衰竭合并肾功能衰竭的疗效以及对心功能的影响。方法将80例慢性心力衰竭合并肾功能衰竭患者随机分为对照组(n=37)与观察组(n=43),对照组采用常规透析方法治疗,观察组采用连续性静一静脉血液透析滤过治疗。比较2组治疗前后呼吸(RR)、心率(HR)、平均动脉压(MAP)、APACHEⅡ评分、Boston心力衰竭积分、血气指标(pH、HCO_3^-、PaO_2、SaO_2)、血生化指标(血肌酐、血尿素氮)及超声心功能指标[二尖瓣口血流速度(SV)、心排出量(CO)、左室射血分数(LVEF)]变化情况。结果 2组治疗后RR、HR、MAP、APACHEⅡ评分、Boston心力衰竭积分、血气指标、血生化指标及超声心功能指标等均较治疗前显著改善,差异均具有统计学意义(P<0.05,P<0.01);观察组HR、APACHEⅡ评分、Boston心力衰竭积分、HCO_3^-、SaO_2、血肌酐、血尿素氮、SV及LVEF水平改善程度显著优于对照组(P<0.05)。结论连续性静一静脉血液透析滤过治疗慢性心力衰竭合并肾功能衰竭的疗效显著,可有效改善心功能。
Objective To explore the effect of continuous veno-venous hemofiltration in treatment of chronic heart failure with renal failure and the influence on cardiac function.Methods Eighty cases of chronic heart failure complicated with renal failure patients were divided into the control group（n = 37） and observation group（n =43）,the control group using conventional dialysis treatment,the observation group with continuous veno- venous hemodiafiltration therapy.Before and after treatment,respirator）＇ rate（RR）,heart rate（HR）,mean arterial pressure（MAP）,APACHEII score,Boston integral of heart failure,blood gas indexes（pH,HCO3^-,PaO2,SaO2）,blood biochemical parameters（serum creatinine,urea nitrogen） and cardiac function index[mitral flow velocity（SV）,cardiac output（CO）,left ventricular ejection fraction（LVEF）]changes were compared between the 2 groups.Results Compared with before treatment,RR,HR,MAP,APACHEII score,Boston heart failure score,blood gas indexes,blood biochemical indexes and cardiac function indexes were significantly improved after treatment in both of the 2 groups,the differences were statistically significant（P〈0.05,P〈0.01）,and the observation group＇s HR,APACHEII score,Boston heart failure score,HCO3^-,SaO2,serum creatinine,blood urea nitrogen,SV and LVEF levels improve significantly belter than the control group（P〈0.05）.Conclusion Continuous veno-venous hemoliltralion in treatment of chronic heart failure with renal failure revealed good therapeutic effect,it can effectively improve heart function.
