摘要
目的:探讨鼻唇沟皮瓣修复较大面积鼻翼缺损的可行性。方法:通过对鼻唇沟区血供及静脉回流的解剖研究,设计鼻唇沟轴型皮瓣,掀起后折叠并旋转至受区修复缺损的鼻翼。结果:1996年至今共治疗外伤性鼻翼缺损3例,面积最大为2.8cm ×2.5cm,最小为2cm ×1.6cm,一次性完成鼻外形及鼻衬里的修复,皮瓣完全成活,均未出现血运障碍,无并发症发生,经4个月～2年随访,效果满意。结论:鼻唇沟皮瓣血运丰富,易成活,可提供足够的组织量用以修复较大面积鼻翼缺损,血管蒂长,转移灵活,供区无明显瘢痕。鼻唇沟折叠皮瓣是修复较大面积鼻翼缺损可选用的最佳皮瓣。
Objective:To explore the feasibility of nasolabial fold's flap major area of pinna nasi defect. Methods: During the vascu-larity in nasolabial fold, we design that nasolabial fold's flap is wrapped and rotated to the defected pinna nasi. Results: From 1996, we have treated three cases of traumatic pinna nasi defect totally. The maximum area was 2. 8cm x 2. 5cm and the minimum was 2cm x 1.6cm. The nose contour and interior were repaired completely only one time, the flap survived totally, and no disturbance of blood circulation or no complication appeared. Between four months and two years follow - up survey, we got the good effect. Conclusion: The nasolabial fold's flap has abundant blood circulation and easily survived. It can also afford ample tissues to repair major area of pinna nasi defect, the blood vessel peduncle was too long to be survived easily, and the giver has not clear scar. The nasolabial fold wrapped flat is the perfectly chosen flap to repair najor area of pinna nase defect.
