以晕厥为首发表现的主动脉夹层误诊为脑血管疾病原因探析 被引量：4

摘要 目的探讨无痛性主动脉夹层(aortic dissection,AD)的临床特点、误诊原因及其防范措施。方法对我院收治的1例曾误诊为脑血管疾病的无痛性AD的临床资料进行回顾性分析。结果本例因突发晕厥5 d,突发剧烈胸痛伴背痛4 d入院。5 d前因晕厥于当地医院神经内科就诊,初步考虑脑血管疾病,予阿司匹林口服及醒脑静注射液静脉滴注等对症治疗,自觉症状好转出院。出院当天于家中进食时突发剧烈胸痛伴背痛,疼痛呈撕裂样,当地医院考虑AD转上级医院,行螺旋CT动脉造影检查示A型AD,但因肝、肾功能异常转入我科,予对症支持治疗,拟病情稳定后行手术治疗,但患者于入科第2天死亡。结论临床接诊以晕厥为首发表现患者时应警惕无痛性AD,要仔细询问病史、及时全面查体、进行相关医技检查,并注重综合分析病情,以减少误诊误治。
