摘要
目的:研究血液净化治疗重症急性胰腺炎(severe acute pancreatitis,SAP)的临床及护理效果.方法:选取2010-06/2013-05唐山市工人医院收治的SAP患者53例,均先采用禁食,胃肠减压等常规方法治疗,并在常规治疗基础上再进行连续性静脉-静脉血液滤过治疗,而且所有患者在治疗过程同时给予针对性护理.治疗后观察及评价疗效.结果:治疗后患者血肌酐(serum creatinine,S C r)、血淀粉酶(blood ureanitrogen,BUN)、脂肪酶(lipase,LPS)、血淀粉酶(blood amylase,AMS)及谷丙转氨酶(alanine transaminase,ALT)水平分别为121.3μmol/L±65.4μmol/L、14.3 mmol/L±4.1 mmol/L、382.1 U/L±131.6 U/L、465.3 U/L±105.4U/L、57.5 U/L±27.6 U/L,治疗后患者这些血生化指标较治疗前有显著性改善(P<0.05).而且患者治疗后多器官功能障碍综合征(multiple organ dysfunction syndrome,MODS)及急性生理与慢性健康(acute physiology and chronic health evaluation,APACHEⅡ)评分也较治疗前有显著性改善(P<0.05).结论:对SAP患者给予血液净化治疗,并在治疗时给予针对性护理的临床效果满意,建议在临床治疗中推广应用.
AIM： To explore the clinical and nursing efficacy in patients with severe acute pancreatitis treated by blood purification. METHODS： Fifty-three patients with severe acute pancreatitis who underwent conventional treatment and continuous veno-venous hemofiltration treatment at our hospital were included, and all patients received targeted nursing. Clinical efficacy was observed. RESULTS： Serum creatinine（SCr）, blood urea nitrogen（BUN）, lipase（LPS）, blood amylase（AMS） and alanine transaminase（ALT） levels after treatment were 121.3 μmol/L ± 65.4 μmol/L, 14.3 mmol/L ± 4.1 mmol/L, 382.1 U/L ± 131.6 U/L, 465.3 U/L ± 105.4 U/L, and 57.5 U/L ± 27.6 U/L, respectively, which were significantly improved compared with the levels before treatment（P 〈 0.05）. Multiple organ dysfunction syndrome（MODS） and acute physiology and chronic health evaluation（APACHE Ⅱ） scores were also significantly improved compared with the values before treatment（P 〈 0.05）. CONCLUSION： Blood purification combined with targeted nursing for treatment of patients with severe acute pancreatitis is effective.
