同型半胱氨酸联合超敏C反应蛋白检测诊断冠状动脉粥样硬化的临床意义 被引量:3

Clinical Significance of Homocysteine and High Sensitive C Reactive Protein on Diagnosis of Coronary Atherosclerosis

摘要 目的探讨同型半胱氨酸(Hcy)和超敏C反应蛋白(hs CRP)联合检测诊断冠心病(coronary heart disease,CHD)的临床意义。方法 220例疑似CHD患者按照冠状动脉造影结果分为CHD组患者143例和非CHD组患者77例,测量并分析两组患者Hcy和hs CRP水平。结果 CHD患者的Hcy和hs CRP明显高于非CHD组,差异有统计学意义(P<0.05),多因素Logistic回归分析显示Hcy、hs CRP均为CHD的危险因素,两指标联合检测显示与CHD呈强相关(OR=14.32,P<0.01),对CHD的诊断准确性为94.09%,Kappa值为0.87(P=1.0)。结论 Hcy联合hs CRP诊断CHD可能有较大的临床意义。 Objective To investigate the clinical significance of homocysteine( Hcy) and high-sensitivity C-reactive protein( hs CRP) on diagnosis of coronary heart disease( CHD). Methods 220 patients who suspected with CHD were divided into CHD group( n = 143) and non-CHD group( n = 77) by coronary angiography. Hcy and Hs CRP standard were measured and analysed. Results Hcy and hs CRP in patients diagnosed with CHD were significantly higher than those of non-CHD group,significance of CHD diagnosis in Hcy and Hs CRP was judged( P < 0. 05). Hcy and hs CRP were risk factors for CHD by multivariate logistic regression analysis,two indexes joint detection showed that there's strong correlation between CHD and joint detection results( OR = 14. 32,P < 0. 001). Diagnostic accuracy was 94. 09% and Kappa was 0. 87( P = 1. 0). Conclusions Hcy combined with hs CRP may have significantly clinical significance on CHD diagnose.
