摘要
目的观察闭合复位空心螺钉固定与切开复位钢板固定治疗跟骨关节内骨折的疗效。方法对我院43例跟骨关节内骨折的患者,在患者知情同意情况下按数字表法随机分成2组:A组采用撬拨闭合复位空心螺钉固定治疗(22例),B组采用切开复位钢板固定治疗(21例),评定2组术后跟骨Bhler角及Gissane角的变化、手术并发症、术中术后出血量、手术和住院时间、骨折愈合后患足功能恢复情况。结果所有患者经0.5~3年随访,均获得骨性愈合。2组在术后功能优良率和Bhler角、Gissane角恢复情况差异均无统计学意义(P〉0.05)。A组在手术及住院时间、术中术后出血量、并发症上明显优于B组,差异有统计学意义(P〈0.05)。结论 2种方法在治疗结果上无明显差别,但空心螺钉固定组具有创伤小、对骨折固定稳定、住院及手术时间短、并发症少、以及经济适用、疗效可靠等特点。
Objective To observe the therapeutic effects of close reduction hollow screw fixation versus open plate internal fixation on calcaneus internal fracture. Methods Forty-three cases of calcaneus internal fractures from our hospital were randomly divided into 2 grnups:group A and group B. Twenty-two feet from group A were treated with kirschner wire percutaneous reduction and close reduction hollow screw fixation, and 21 feet from group B were treated with open plate internal fixation. Following surgery, assessment was made in changes in B? hler's and Gissane's angles, surgical complications, the amount of blood loss during and after surgery, the durations of surgery and hospitalization and recovery of the foot function after healing of the bone fracture. Results All the patients had O. 5 - 3-year medical follow-ups and results indicated that they all had bony union. No statistical significance could be noted in good recovery rate and the recovery of Bohler's and Gissane's angles(P 〉 0. 05 ). However, marked statistical significance could be found in the durations of surgery and hospitalization, the amount of blood loss during and after surgery, as well as complications, when comparisons were made between the 2 groups ( P 〈 0. 05 ). Conclusion No obvious differences could be noticed in the therapeutic effects of the 2 treatment methods. However, open plate internal fixation had the features of minimal invasion, firm fixation, short hospital stay and surgical time, less complications, easy applicability and more reliable therapeutic efficacy.
