摘要
目的:探讨双侧平衡去骨瓣减压术治疗重型闭合性颅脑损伤的临床效果及安全性。方法:回顾性分析2011年5月至2014年10月期间来我院就诊的重型闭合性颅脑损伤患者118例,将患者分为两组,双侧组(采用双侧平衡去骨瓣减压术治疗)及单侧组(采用标准去骨瓣减压术治疗),每组59例,比较两组颅内压、生存质量及并发症发生情况。结果:术后1d及术后3d双侧组及单侧组患者颅内压与治疗前比较均出现明显下降,双侧组术后1d及术后3d颅内压与单侧组比较均明显较低,两组差异有统计学意义(P<0.05)。双侧组恢复良好率(35.59%)与单侧组(16.95%)比较明显较高,重度残疾率(16.95%)、死亡率(5.08%)及并发症总发生率(11.86%)与单侧组(25.42%、22.03%、30.51%)比较均明显较低,两组差异有统计学意义(P<0.05)。结论:重型闭合性颅脑损伤采用双侧平衡去骨瓣减压术治疗疗效确切,可快速降低颅内压,使患者术后生存质量提高,并发症发生率降低,有较高的安全性。
Objective： To study the clinical efficacy and safety of bilateral balance to bone disc decompressive craniectomy in the treatment of severe closed craniocerebral injury.Methods：A retrospective analysis in May 2011 to October 2014 to our hospital during the period of 118 patients with severe closed craniocerebral injury were divided the patients into two groups,group on both sides （double side balance to bone disc decompression treatment） and unilateral group （with a standard to bone disc decompression treatment）,59 cases in each group,compared two groups of intracranial pressure,the quality of survival,and complications.Results：Postoperative 1 d and 3 d after bilateral and unilateral group patients with intracranial pressure occurred significantly decreased compared with before treatment,bilateral group 1 d and 3 d after operation,the postoperative intracranial pressure compared with unilateral group were significantly lower,the difference between the two groups was statistically significant （P 〈 0.05）.Bilateral group good recovery rate （35.59%） significantly higher compared with unilateral group （16.95%）,severe disability （16.95%） and mortality （5.08%） and total incidence of complications （11.86%） with unilateral group （25.42%,22.03%,30.51%） were significantly lower,the difference between the two groups was statistically significant （P 〈 0.05）. Conclusion：Severe closed craniocerebral injury with bilateral balance to bone disc decompression treatment curative effect,can quickly reduce the intracranial pressure,improved the quality of patients with postoperative survival,reduce the incidence of complications,higher security,can be used as a treatment for severe closed craniocerebral injury of a kind of effective method in clinical application.
