摘要
目的探讨双水平气道正压无创通气治疗老年慢性阻塞性肺疾病合并呼吸衰竭患者的临床效果。方法对安阳市人民医院收治的42例老年慢性阻塞性肺疾病合并呼吸衰竭患者在实施常规治疗上应用双水平气道正压无创通气（BIPAP）治疗，观察患者治疗前后各项血气分析指标情况并对其进行比较。结果经治疗后，42例患者的临床症状均有所改善，且患者治疗后的pH、二氧化碳分压、动脉血氧分压及血氧饱和度等指标与治疗前比较均有显著性改善，差异有统计学意义（P〈0．05）。结论老年慢性阻塞性肺病合并呼吸衰竭患者应用双水平气道正压无创通气治疗可使其呼吸困难、嗜睡等症状及血气分析指标明显改善，避免气管插管上呼吸机，值得临床推广应用。
Objective To observe the clinical effect of double level noninvasive positive airway venti- lation on elderly patients with chronic obstructive pulmonary disease with respiratory failure. Methods Forty- two cases of senile patients with chronic obstructive pulmonary disease with respiratory failure patients admit- ted to our hospital were chose as the research objects. Clinical data were collected and analyzed retrospective- ly. All patients were given conventional treatment with non-invasive bi level positive airway pressure ventila- tion（BIPAP） therapy. The blood gas indexes before and after treatment were observed and compared. Re- suits After treatment, the clinical symptoms of 42 patients were improved. And after treatment, the pH, partial pressure of carbon dioxide PCO2 （mm Hg）, arterial partial pressure of oxygen PaO2 （mm Hg）, and arterial oxygen saturation （SaO2 ） were compared with before treatment were significantly improved, there were significant differences （P 〈 0. 05）. Conclusions The application of non-invasive bi level positive air- way pressure ventilation in elderly patients with chronic obstructive pulmonary disease with respiratory fail- ure can releive breathing difficulties, symptoms of sleepiness and improve blood gas analysis, avoid endo- tracheal intubation ventilator, has higher application value, so it is worthy of clinical application.
