摘要
目的比较分别用D-dimer PLUS(DDP)和D-dimer INNOVANCE(DDI)两种试剂检测血浆D二聚体含量的差异,并分析以D二聚体检测值为依据对临床干预(实施抗凝治疗)的适宜参考范围,为临床应用提供依据。方法1分别用DDP和DDI两种试剂检测同一批血浆标本(包括正常和异常)的D二聚体含量,分析相关性。2回顾性研究.随机抽取大连医科大学附属第一医院2011-2012年用DDP试剂测得D二聚体值的患者242例。用DDI试剂测得的D-二聚值的患者244例,通过查阅病案的方式观察每例是否应用抗凝药肝素或低分子肝素等进行临床干预,观察两种试剂检测的D二聚体水平与临床干预值的关系。结果 DDP与DDI两种试剂检测D二聚体结果有相关性,相关系数r=0.51,P<0.05。DDP组在1000-1500μg/L DDU范围进行临床干预人数为62.5%,而DDI组在5000-6000μg/L FEU范围进行临床干预人数65%。可见,用DDP试剂测得的D二聚体的1000-1500μg/L DDU大致相当于DDI试剂测得的D二聚体值的5000-6000μg/L FEU。结论 DDP和DDI两种试剂检测血浆D二聚体值差异很大,后者是前者的5倍。以D二聚体为依据实施的临床干预值也明显不同,建议临床医生在应用D二聚体指标时一定要注意D二聚体检测所使用的试剂盒的种类,以便合理用药。
Objective To compare the differences of plasma D-dimer content detected by D-Dimer PLUS（DDP） or D-Dimer INNOVANCE （DDI）reagents, to analyze the effect of D-dimer on clinical interven- tion value（ the percentage of implementation of anticoagulant therapy） , to seek the appropriate treatment refer- ence range to provide the basis for clinical application. Methods （1）With DDP respectively and DDI two rea- gent detection with a batch of plasma samples （ including normal and abnormal） D- Dimer content,, analysis correlation. （2） The retrospective study. The cases were selected from the First Affiliated Hospital of Dalian Medical University during 2011 to 2012. 242 samples were detected by DDP while another 244 samples were detected by DDI. By searching the medical records,we record whether the clinical intervention were given to the patients in the two groups to analyse the difference of clinical intervention values. Results D-direct levels detected by DDP or DDI are comparable,r = 0.51 ,P 〈 0.05. The clinical intervention percentage is 62.5% in the range of 1000-1500 μg/L DDU in DDP group ,while the clinical intervention percentage is 65% in the range of 5000-6000 μg/L FEU in DDI group. Conclusion The plasma D-dimer levels detected by DDP or DDI vary widely, and the D-dimer level detected by DDP is five times more than DDI. Clinical intervention value based on the D-direct level is different significantly. It suggests that clinical doctor must be pay attention to specie of D-Dimer detection kit when they prescribe.
