生物反馈电刺激对子宫切除术后盆底功能障碍性疾病的意义 被引量:36

Effect of biological feedback combined with electrical stimulation in the treatment of female pelvic floor function after survey of total hysterectomy

摘要 目的:研究生物反馈电刺激治疗对改善全子宫切除术后盆底功能障碍的效果。方法:收集2013年2月-2014年2月石河子大学医学院第一附属医院妇产科因子宫良性疾病行全子宫切除术的并经术后诊断合并压力性尿失禁(SUI)的患者160例,随机分为两组,治疗组80例给予生物反馈电刺激+盆底肌肉锻炼(Kegel锻练),对照组80例给予单纯Kegel锻练,另取80例正常患者作为正常对照组,术后1个月与术后6个月分别进行随访调查,调查项目包括盆底肌力测试,盆底三维B超,尿动力学检测,盆底障碍影响简易问卷7(PFIQ-7)、盆腔脏器脱垂/尿失禁性功能问卷(PISQ-31),评价盆底功能锻炼的效果。结果:子宫切除术后1个月尿失禁(UI)患者的盆底肌力、尿动力学参数及盆底超声参数均与正常对照组差异有统计学意义(P〈0.05);治疗组与对照组患者术后6个月盆底肌收缩力、UI症状均有所改善,但治疗组效果更明显,差异有统计学意义(P〈0.05);两组治疗后盆底超声参数膀胱颈移动度、膀胱颈下移至耻骨联合下距离和尿道膀胱后角均减小,但治疗组效果更明显,且差异有统计学意义(P〈0.05)。结论:生物反馈电刺激治疗可更有效改善女性术后盆底功能障碍的症状,且具有操作简便,应用方便,无不良反应的特点,值得临床广泛推广使用,但实际应用中应强调个体化治疗原则及患者主动参与配合治疗。 Objective: To investigate effect of biological feedback combined with electrical stimulation in the treatment of female pelvic floor function after survey of total hysterectomy. Methods: Benign uterine disease with total hysterectomy and patients with postopera- tive diagnosis merger of 160 cases of stress urinary incontinence from Obstrics and Gynecology in First Affiliated Hospital of Xin Jiang Shihezi University School of Medicine between February 2013 and February 2014 were collected, who were randomly divided into two groups. 80 ca- ses were treated with biological feedback combined with electrical stimulation and kegel treatment, the remaining were treated with kegel treatment, another 80 cases of normal patients as controls. The information of pelvic floor mutscle strength, urodynamic and three-dimension- al ultrasound parameters, Pelvic Floor Inventory Questionnaire (PEIQ-7) and POP/UI Sexual Function Questionnaire (PISQ-31) were collected. The effect of pelvic floor exercises was calculated. Results : The patients after a month of survey of total hysterectomy differed from the normal group about pelvic floor muscle strength, urodynamic mid three-dimensional ultrasound parameters with statistically significant. ( P〈O. 05) The pelvic floor muscle strength of treatment group was higher than that of control group, while urodynamic and three-dimension- al ultrasound parameters were lower than those of control group. Urinary incontience symptoms of both two groups were improved, and effects of the treatment group were obvious. Conclusion: Biological feedback combined with electrical stimulation treatment is more effective than kegel treatment, is easy to play, has no side effecs, deserves to used in clinical, but individualized treatment principles and active partici- pation of patients are needed.
