重度妊娠高血压病对早产儿白细胞及凝血功能影响的研究 被引量:10

摘要 目的观察重度妊娠高血压病(HCDP)对早产儿白细胞及凝血功能的影响。方法选取2011年1月至2014年12月收治的患有HCDP母亲所产的早产儿为观察组,对照组为健康母亲所产的早产儿。比较两组的血常规及凝血功能的特点、变化及影响因素,比较两组常见早产儿并发症的差异。结果观察组胎龄、出生体重小于对照组,SAG发生率高于对照组(P〈0.05)。观察组生后6 h、48~72 h、144 h 3个时间段内白细胞[白细胞总数(WBC)、中性粒细胞绝对数(ANC)]均低于对照组(P〈0.01)。观察组在生后6 h、48~72 h、144 h 3个时间段内的血小板(PLT)均低于对照组(P〈0.05)。观察组在生后6 h、48~72 h、144 h 3个时间段内的凝血功能:凝血酶原时间(PT)、部分活化凝血酶原时间(APTT)、血浆凝血酶时间(TT)均高于对照组(P〈0.05);纤维蛋白原(FIB)低于对照组(P〈0.05)。两组间新生儿坏死性小肠结肠炎、呼吸窘迫综合征、支气管-肺发育不良(BPD)的发生率、病死率进行对比,差异无统计学意义(P〉0.05);在观察组中,脑出血、肺出血、消化道出血、感染发生率高于对照组,差异有统计学意义(P〈0.05)。结论母亲患有重度HCDP,则所产的早产儿较健康母亲所产的早产儿更易发生白细胞、ANC、血小板减少及凝血功能障碍,并容易引起感染及合并出血。 Objective The aim of this study is to investigate the effect of severe pregnancy induced hypertension on the leukocyte and blood coagulation function of premature infants. Methods From January 2011 to December 2014, the premature infants treated in the neonatal intensive care unit were observed in the observation group. The control group was treated with the healthy mother in our hospital. Changes and influence factors of characteristics were observed and compared in blood routine and blood coagulation function, The complications of premature infants in the two groups were compared. Results The birth weight in observation group is less than the control group. The SAG of the observation group higher than the control group ( P 〈 0.05 ). The WBC ( including WBC and ANC) in the observation group was observed at 6 h, 48 - 72 h, and 144 h. And the ceils were lower than those in the control group ( P 〈 0. 01 ). The PLT in the observation group was observed at 6 h, 48 - 72 h, 144 h. The PLT was lower than the control group ( P 〈 0.05 ). The coagulation function of the observation group was observed at 6 h, 48 - 72 h, 144 h. The PT, TT, APT/" were higher than those of the control group ( P 〈 0.05 ). FIB was lower than the control group ( P 〈 0.05 ). The incidence and mortality of necrotizing colitis, respiratory distress syndrome, BPD were compared between the observation group and control group ( P 〉 0.05 ). In the observation group, cerebral hemorrhage, pulmonary hemorrhage, digestive tract bleeding, infection rate was higher than the control group, with statistically significant ( P 〈0.05). Conclusion Mother suffering from severe HCDP produced preterm infants, who had high incidence of of WBC, ANC, thrombocytopenia, and dysfunction of blood coagulation, easily lead to infection and hemorrhage.
