选择性头部亚低温治疗HIE的疗效及护理

Curative Effect and Nursing of Selective Head Cooling with Mild Systemic Hypothermia in the Treatment of Hypoxic Ischemic Encephalopathy

摘要 目的探讨选择性头部亚低温治疗新生儿缺血缺氧性脑病(HIE)的疗效及护理。方法选取2013年1月至2013年12月我院收治的HIE足月儿78例为研究对象,将患儿随机分为观察组和对照组,每组39例。对照组患儿接受常规治疗及护理,肛温维持为36.7℃~37.5℃,观察组在对照组的基础上,于出生后6 h内加用选择性头部亚低温治疗及护理。对两组患儿的疗效进行比较分析。结果治疗1周、2周、4周后,对照组患儿的NBNA评分均有提高,与治疗前比较差异有统计学意义(均P〈0.05);观察组NBNA评分亦有显著提高,与治疗前比较差异有统计学意义(分别P〈0.05,P〈0.01,P〈0.01);两组间治疗后比较差异有显著统计学意义(P〈0.01)。结论选择性头部亚低温疗法并配合有针对性的护理,能够有效保护脑组织,对降低HIE的致残率具有重要的意义。 Objective To explore the efficacy and nursing points of selective head cooling with mild systemic hypothermia for treatment of neonatal hypoxic ischemic encephalopathy(HIE). Methods 78 cases of term infant with HIE admitted to our hospital from January 2013 to December 2013 were selected and randomly divided into the control group and the observation group equally. All infants were given routine therapy and nursing; in addition, infants in the observation group were given selective head cooling with mild systemic hypothermia within 6 h after birth. The curative effects of two groups were observed and compared. Results At 1stweek, 2ndweek and 4th week after therapy, the NBNA scores of control group increased significantly(all P 0.05); the NBNA scores of observation group increased more significantly(P 0.05, P 0.01, P 0.01), which obviously higher than those of control group at the same time point(all P 0.01).Conclusions The treatment of selective head cooling with mild systemic hypothermia combined with targeted care for neonatal HIE can effectively protect brain tissue, which has great significance for reducing the disability rate of HIE.
