急性缺血性脑卒中患者脑源性神经营养因子基因多态性与预后的相关性 被引量:12

Correlation of brain-derived neurotrophic factor gene polymorphism with prognosis in patients with acute ischemic stroke

摘要 目的探讨急性缺血性脑卒中(AIS)患者脑源性神经营养因子(BDNF)基因多态性与预后的关系。方法选择2011年9月-2014年5月该院收治的290例AIS患者为研究组,同期在该院体检的健康者200例为对照组,抽取各组受试者静脉血5 ml,提取基因组DNA,应用聚合酶链式反应(PCR)和连接酶检测反应(LDR)检测BDNF基因G196A位点多态性,计算研究组与对照组基因频率,并分析基因型与预后的关系。结果研究组与对照组性别构成、年龄比较差异无统计学意义(P>0.05),研究组高血压、糖尿病、高血脂发生率显著高于对照组(P<0.05)。研究组G/G型、G/A型、A/A型基因频率分别为21.0%、50.0%和29.0%,对照组分别为28.5%、56.0%和15.5%,研究组A/A型基因频率显著高于对照组(P<0.05);研究组A基因频率亦显著高于对照组(P<0.05)。按照改良Rankin量表评分,研究组预后良好182例,预后差108例;其中BDNF基因G196A位点中A/A基因型携带者预后不良发生率显著升高(P<0.05)。结论 BDNF基因G196A位点中A/A基因型能够增加汉族人群AIS的易感性,携带A/A基因型的AIS患者预后更差,提示A等位基因可以作为AIS预后不良的生物学指标。 [Objective] To explore the relationship between brain-derived neurotrophic factor (BDNF) gene polymorphism and prognosis of acute ischemic stroke (AIS) patients. [Methods] A total of 290 cases of acute ischemic stroke treated in our hospital from September 2011 to May 2014 were selected as study group, and 200 people who had physical examination during the same period were selected as control group. Venous blood 5 ml was extracted in each group. Genomic DNA was extracted. BDNF gene G196A polymorphism was detected with polymerase chain reaction (PCR) ligase detection reaction (LDR) method. Gene frequency of both groups was calculated, and the relationship between the genotype and prognosis was analyzed. [Results] The gender, age and other data of the two groups had no statistical differences (P〉 0.05). The incidences of hypertension, diabetes and high blood lipid in the study group were significantly higher than those in the control group (P〈0.05). The G/G type, G/A type and A/A type gene frequencies were 21.0%, 50.0% and 29.0% respectively in the study group, and 28.5%, 56.0% and 15.5% respectively in the control group. The A/A gene frequency in the study group was significantly higher than that in the control group (P〈 0.05). According to the modified Rankin scale score, there were 182 cases with good prognosis and 108 cases with poor prog- nosis in the study group. The incidence of poor prognosis significantly increased in the patients with A/A genotype of BDNF gene G196A locus (P〈O.05). [Conclusions] A/A genotype of BDNF gene G196A locus can increase the susceptibility of acute cerebral ischemic stroke in Han population, and AIS patients carrying A/A genotype have worse prognosis, which indicate that the A allele may be used as a biological marker for poor prognosis of AIS.
