摘要
目的应用Endo-PAT 2000无创血管内皮检测系统评估冠心病患者的血管内皮功能及其影响因素。方法连续入选本院心内科患者183例,其中冠心病组126例,冠脉造影正常的对照组57例。以EndoPAT 2000测定的反应性充血指数(reactive hyperemia index,RHI)作为评价冠心病患者内皮功能的主要指标,采用多重线性回归分析RHI的影响因素。结果冠心病组患者RHI平均为(1.58±0.39),对照组为(1.70±0.24),差异有显著性(P<0.05)。多重线性回归分析显示,RHI与吸烟、空腹血糖、糖化血红蛋白、高敏C反应蛋白水平、冠心病危险因素的数量等相关,差异有显著性(P均<0.05)。结论冠心病患者的血管内皮功能障碍明显,RHI与吸烟、空腹血糖、糖化血红蛋白、高敏C反应蛋白及冠心病危险因素的数量有显著的相关性。
Objective This study was undertaken to evaluate vascular endothelial function and various influencing factors in patients with coronary heart disease( CHD) by using the Endo-PAT2000 Noninvasive Diagnostic System. Method A total of 183 consecutive patients treated at the Department of Cardiology were enrolled. They were divided into a CHD group( n = 126),no-CHD group( n = 57). The reactive hyperemia index( RHI) was determined with the Endo-PAT2000 Noninvasive Diagnostic System. The reactive hyperemia index( RHI) was used as a main indicator to assess the endothelial function of the patients with coronary heart disease. Multiple linear regression analysis was used to analyze the correlations between RHI and various influencing factors. Result The results of Endo-PAT test showed that mean RHI was(1. 58 ± 0. 39) in patients with coronary heart disease,and it was(1. 70 ± 0. 24) in the control group,there was significant difference between the two groups( P〈0. 05). Multiple linear regression showed that RHI was correlated with smoking history,fasting blood glucose,hemoglobin A1 c,and high-sensitivity C-reactive protein,RHI is also related to the number of risk factors for CHD( P 0. 05). Conclusion Endothelial function decreased in patients with coronary heart disease. Smoking,FBG,Hb A1 c,and hs-CRP are significantly associated with endothelial dysfunction. The endothelial dysfunction is also related to the number of risk factors for CHD.
