阳性血培养物直接涂片革兰染色镜检与培养结果的相关性 被引量:11

Correlation between the direct Gram staining smear detection and the culture results in positive blood cultures

摘要 目的分析阳性血培养物涂片革兰染色油镜下细菌染色性、形态与培养结果的相关性。方法对本院2012年7月-2014年7月临床送检的447份经梅里埃公司Bac T/ALERT 3D型血培养仪报警阳性的需氧血培养瓶肉汤进行涂片革兰染色,油镜下观察细菌染色性、形态特征,同时进行细菌分离培养与鉴定,比较2种方法结果的一致性。结果血培养物涂片细菌染色性与培养结果的符合率为98.4%(440/447)。葡萄球菌、肠球菌/链球菌、肠杆菌科细菌、假单胞菌及相关菌的镜下形态与培养结果的阳性符合率分别为99.6%、100.0%、96.4%、92.9%,革兰染色镜检的敏感度分别为98.8%、95.2%、95.7%、86.7%,特异度分别为99.5%、100.0%、98.3%、99.8%。结论阳性血培养物涂片革兰染色镜检细菌染色性、形态与培养结果的符合率高,微生物实验室向临床报告阳性血培养染色结果时应准确描述细菌染色性及形态,为临床早期诊断与经验性治疗血流感染提供重要信息。 Objective To analyze the correlation among the Gram stain reactions, morphologic type of bacteria and culture results in positive blood cultures of the Gram staining smear under the oil immersion lens. Methods In our hospital from July 2012 to July 2014, 447 aerobic blood culture bottle broth smear Gram staining from clinical inspection, which was detected positive with Merieux BacT/ALERT 3 D blood culture instrument, was performed under oil microscope for the observation of bacterial staining, morphology, bacteria isolation, and for culture and identification at the same time, so as to compare the results consistency of the 2 methods. Results The coincidence rate of the Gram stain reactions and culture results was 98.4%. The positive coincidence rates of the morphology and culture results for Staphylococci, Enterococci/Streptococci, Enterobacteriaceae, Pseudomonas and related organisms were 99.6% , 100.0% , 96.4% and 92.9%, respectively. The sensitivities of the Gram stain were 98.8%, 95.2% , 95.7% and 86.7%, respectively, the specificities were 99.5%, 100.0%, 98.3% and 99.8% , respectively. Conclusion The coincidence rate of bacteria staining, morphology and culture results in the positive blood culture smear Gram staining microscopic examination is high. The microbiology laboratory, when reporting the clinical positive blood culture staining results, should accurately describe bacterial staining and morphology, so as to provide important information for the clinical early diagnosis and empirical treatment of blood infection.
