摘要
目的 探讨白介素-6(IL-6)及生存素(Survivin)在胚胎停育中的临床意义,为胚胎停育的诊断和治疗提供理论依据。方法 选取2014年1-10月该院产科门诊接诊的不明原因胚胎停止发育患者56例为胚胎停育组,另选取同期正常早孕吸宫流产患者56例为正常早孕组、正常未孕女性42例为未孕组,对各组的血清及绒毛组织进行检测分析。采用RT-PCR和SP法检测绒毛组织中IL-6、Survivin mRNA的表达、蛋白定位及表达强度;采用ELISA法测定IL-6在3组研究对象血清中的浓度。结果 在绒毛组织中,IL-6与Survivin在合体及绒毛滋养层细胞中均有表达,RT-PCR及SP结果均显示IL-6与Survivin在胚胎停育组中的表达水平均低于正常早孕组,差异有统计学意义(P〈0.05);ELISA法检测胚胎停育组中IL-6表达低于正常早孕组和未孕组,但差异无统计学意义(P〉0.05)。结论 孕早期绒毛组织中IL-6和Survivin的正常表达对维持早期胚胎发育的正常进行具有重要意义,IL-6可通过Stat3信号转导通路而对Survivin的表达进行有效调节,IL-6和Survivin的低表达可能引发稽留流产。
Objective To explore the clinical significance of interleukin- 6( IL- 6) and Survivin in embryo growth arrest,provide theoretical basis for diagnosis and treatment of embryo growth arrest. Methods From January to October in 2014,56 patients with unexplained embryo growth arrest were selected as embryo growth arrest group from Obstetric Outpatient in the hospital,56 patients after abortion during early pregnancy were selected as normal pregnancy group,42 normal unpregnant women were selected as unpregnancy group. The serum and chorionic villi samples in the three groups were detected and analyzed. RT- PCR and immunohistochemical SP method were used to detect the expressions of IL- 6 and Survivin mRNA,protein localizations,and expressional intensities in chorionic villi samples; ELISA was used to detect the serum concentrations of IL- 6 in the three groups. Results In chorionic villi samples,IL- 6 and Survivin expressed in syncytiotrophoblast and villous trophoblast cells,RT- PCR and immunohistochemical SP method showed that the expression levels of IL-6 and Survivin in embryo growth arrest group were statistically significantly lower than those in normal pregnancy group( P〈0. 05); ELISA showed that the expression level of IL- 6 in embryo growth arrest group was lower than those in normal pregnancy group and unpregnancy group,but there was no statistically significant difference( P〉0. 05). Conclusion Normal expressions of IL- 6 and Survivin in chorionic villi during early pregnancy have important significance in maintaining normal and early embryonic development,IL- 6 can effectively regulate Survivin expression via Stat3 signal transduction pathway,low expressions of IL- 6 and Survivin may lead to missed abortion.
