摘要
目的探讨神经肌肉电刺激治疗对脑梗死吞咽障碍患者焦虑抑郁状态的影响。方法选取有吞咽障碍的脑梗死患者126例,洼田氏饮水试验吞咽功能III级(52例)、IV级(45例)、V级(29例),各级之间进行Hamilton焦虑量表及Hamilton抑郁量表测试比较。126例患者再随机分为治疗组(66例)和对照组(60例)均给予常规药物治疗及吞咽训练,治疗组加用神经肌肉电刺激治疗,治疗前后均给予Hamilton焦虑量表及Hamilton抑郁量表测试。结果患者焦虑、抑郁评分与吞咽障碍的严重程度呈相关性,吞咽障碍程度越严重者,焦虑、抑郁越严重,并经过平均2个疗程的分组治疗,与治疗前及对照组比较,治疗组Hamilton焦虑量表中躯体性焦虑、精神性焦虑及总分有不同程度改善(P<0.05),Hamilton抑郁量表中焦虑、体质量、认识障碍、阻滞、绝望感及总分有不同程度改善(P<0.05)。结论脑梗死患者焦虑、抑郁程度与吞咽障碍的严重程度有相关性,神经肌肉电刺激治疗可以改善脑梗死患者的焦虑抑郁状态。
Objective To study the effect of neuromuscular electrical stimulation to anxiety-depression in patients with cerebral infarction. Methods 126 patients with post-stroke dysphagia, swallowing function divided into Ⅲ class ( n = 52), Ⅳ class ( n = 45 ), Ⅴ class ( n = 29) according to via the water swallow tests, Patients were evaluated with scores of Hamilton anxiety rating scale (HAMA), Hamilton depression (HAMD)at different levels, then 126 patients with post - stroke dysphagia were randomized into the treatment group (n = 66) and the control group (n = 60). Patients in two groups received conventional drug therapy and swallowing function training, while the patients in the treatment group received NES in addition. Patients were evaluated with scores of Hamilton anxiety rating scale (HAMA) and Hamilton depression (HAMD) before and after treatment. Results The scores of depression and anxiety were positively correlated swallowing dysfunction severity. After treatment for two courses , somatic anxiety, psychic anxiety and the total of the treatment group in HAMA were improved (P〈 0.05 ), anxiety, body weight, cognitive disturbance, inhibition, hopeleness the total of the treatment group in HAMD were improved (P〈0.05) than those before treatment and those in control group. Conclusion Depression and anxiety of patients with post- stroke dysphagia were positively correlated swallowing dysfunction severity. Neuromuscular electrical stimulation applied in patients with cerebral infarction could improve anxiety-depression.
