摘要
目的分析医疗保险制度改革(简称医保改)对医院糖尿病患者住院药品费用的影响。方法选取医院2013年5月至2014年5月期间内分泌科出院或转其他科诊治后出院的糖尿病住院患者1 400例(A组),2008年(即医保改元年)糖尿病住院患者820例(B组),对比分析患者性别、年龄、有无社会医疗保险、住院天数、有无并发症等影响因素。结果对住院药品费用的影响效果由大到小的因素依次是有无并发症、住院天数、有无保险和年龄,对日均住院药品费用的影响效果是有无并发症、住院天数、有无保险和疗效。A组与B组比较,平均住院天数减少,有社会医疗保险比例为90.93%和75.20%,无社会医疗保险比例为9.07%和24.80%,差异有统计学意义(P<0.05)。结论本研究中有医疗保险的患者比例占到90%,与2008年相比住院天数减少,药品费用增长并不明显,医疗保险制度改革后,为中国居民带来的保障值得肯定。
Objective Analysis of the influence of the reform of medical insurance system in hospital drug expenses of patients with di-abetes in our hospital. Methods Select the data of 1 400 cases （A group） of that hospital department of Endocrinology, discharged or transferred to other hospital diabetes hospitalized patients in our hospital in May 2013 to May 2014. Factors affecting the statistical pro-cessing and analysis of hosp - italized patients with drug costs： gender, age, social medical insurance, hospital stay, complica-tion. Comparative analysis of the 2008 statistical data of 820 cases （B group）. Results On the factors influencing the effect of hospital drug costs are complications, length of stay, there is no insurance, age. Effect on the average hos - pitalization cost by the major factors to the complications, length of hospital stay, there is no insurance, curative effect. The average length of A group has reduced than B group, social medical insurance patients was 90. 93% in A group, 75. 20% in B group, the number of patients without social medical insurance is 9. 07% vs. 24. 80% （ P〈 0. 05） . Conclusion The study of medical insurance patients accounted for 90% , compared to 2008 decreased length of hospital stay. The increase of drug expenses is not obvious. The reform of medical insurance system for Chi-nese residents, the guarantee is worthy of recognition.
