摘要
目的研究急性脑梗死老年患者洋葱伯克霍尔德菌感染的临床特点和耐药性,为合理选用抗菌药物治疗洋葱伯克霍尔德菌感染提供依据。方法选取2010年1月-2013年12月洋葱伯克霍尔德菌感染的74例老年患者为研究对象,对所有患者感染洋葱伯克霍尔德菌的分布和耐药性进行回顾性分析,采用SPSS 15.0软件进行统计分析。结果 74例急性脑梗死洋葱伯克霍尔德菌感染老年患者送检标本主要分布在痰液中,共57株占77.0%;洋葱伯克霍尔德菌对头孢他啶的耐药率为47.3%,对磺胺甲噁唑/甲氧苄啶的耐药率为8.1%;对头孢他啶耐药性明显高于米诺环素、美罗培南和磺胺甲噁唑/甲氧苄啶。结论急性脑梗死长期住院的老年患者洋葱伯克霍尔德菌感染临床上主要分布在痰液中,少量见于血液中;对米诺环素、美罗培南基本无耐药性,而对头孢他啶有较高的耐药性。
OBJECTIVE To study the clinical characteristics of the elderly acute cerebral infarction patients with Burkholderia cepacia infection and analyze the drug resistance so as to reasonably use antibiotics for treatment of B .cepacia infection .METHODS A total of 74 elderly patients with B .cepacia infection who were treated from Jan 2010 to Dec 2013 were recruited as the study objects ,then the distribution and drug resistance of the B .cepa‐cia causing infection were retrospectively analyzed ,and the statistical analysis was performed with the use of SPSS 15 .0 software .RESULTS The sputum specimens were dominant among the submitted specimens that were ob‐tained from the 74 elderly acute cerebral infarction patients with B .cepacia infection ,and totally 57 （77 .0% ） strains of B .cepacia were isolated .The drug resistance rate of all the B .cepacia strains to ceftazidime was 47 .3% ,the drug resistance rate to sulfamethoxazole‐trimethoprim was 8 .1% ,and the drug resistance rate was significantly higher to ceftazidime than to minocycline , meropenem , and sulfamethoxazole‐trimethoprim . CONCLUSION B .cepacia strains causing infection in the elderly patients with acute cerebral infarction are mainly isolated from the sputum specimens ,and few strains are isolated from the blood specimens .The Burkholderia ce‐pacia strains are highly susceptible to minocycline and meropenem but were highly resistant to ceftazidime .
